ATI PHYSICAL THERAPY WEBSITE SCAVENGER HUNT Official Rules & Prize Acceptance Form NO PURCHASE NECESSARY

1. ENTRY: Register to win by visiting the ATI Website Scavenger Hunt link. Enter your answers, name, and email address on the form. You are allowed one entry per day, per person. One (1) Grand Prize Winner will be drawn from all correct entries received following the conclusion of the contest at midnight on March 15, 2013. The prize Winner will be notified via email that he or she has won. Odds of winning will be determined by the number of entries in the contest. 2. ELIGIBILITY: Participation is open to legal residents of the Continental United States, ages 18 years of age. Parents or legal guardians of participant must enter on behalf of minor and must be 18 years of age or older. We do not knowingly or intentionally collect personal information from children under the age of 18 and will automatically reject and delete the entry of any person we believe in good faith to be underage. Any false or fraudulent registration or entry from a minor will be deemed to be ineligible and will disqualify the minor from the receipt of a prize or participation. Employees of ATI Physical Therapy and their immediate family members (spouse, parent, child, sibling and their respective spouses, regardless of where they reside) and those living in the same household of each are not eligible to enter or win. Void where prohibited or restricted by law and subject to all federal, state and local laws and regulations. 3. LIMITATIONS: All participants agree to be bound by these rules. If unable to contact a prize winner, after a reasonable effort has been made during two (2) business days from the first notification attempt, such person may be disqualified with an alternative winner selected. Winner must be able to pick up the prize at an ATI Physical Therapy clinic closest to their place of residence. If the winner is unable to meet contest limitations, they will forfeit the prize. 4. PRIZES: One (1) Grand Prize Winner will be drawn at random from all correct entries. Prize schedule is listed below. All prizes are nontransferable with no cash substitutions. One (1) ATI Prize Package 5. CLAIMING OF PRIZES: Prize Winner will be required and agrees to complete and sign the Official Rules and Prize Receipt Form. Winner must be able to pick up the prize at an ATI Physical Therapy clinic closest to their place of residence. If the winner is unable to meet contest limitations, they will forfeit the prize. 6. RESERVATIONS: Each entrant by entering this giveaway agrees: (1) to be bound by these rules and the judges decisions; (2) that none of the Entities, nor any of their officers, directors, employees, agents or representatives are responsible for claims, injuries, losses or damages of any kind resulting from giveaway participation or the acceptance, use, misuse, possession, loss or misdirection of any prize.

7. RELEASES: In this giveaway, are providing your information to ATI Physical Therapy and not to Facebook. Entrant releases personal information provided to the business where registered. Personal information will be used to implement the specific activity or event and will be used for the marketing of additional events and services and merchandising for ATI Physical Therapy. Entrant will have the option to be excluded from future contact, and opt out request information will be included on all future marketing and contact material. We will not sell, exchange, rent, lease or otherwise disclose your personal information to other third-party marketers or any such persons that are not associated with this specific event activity. Participation constitutes permission to ATI Physical Therapy, without any further obligation to the contest winner, to use winner's name, address and likeness for purposes of advertising and trade, except where prohibited by law. Entrant and Winner agrees to release and hold harmless ATI Physical Therapy, and their parent, subsidiary or affiliated companies, prize suppliers, advertising and promotion agencies, and any subcontractors, employees, officers, directors, agents and representatives (the "Releases") of any of the above organizations, from and against any and all claims for losses or damages sustained by said winners, arising out of participation in the giveaway or acceptance or use of the prize. 8.
