Union College's performance appraisal process provides an opportunity for employee and supervisor to discuss job performance relative to outcomes desired and other performance criteria. An important part of this process is the Self Assessment. The Self Assessment helps to ensure greater perceived accuracy, fairness and improved understanding of position demands and expectations. A completed Self Assessment Worksheet and a Performance Evaluation Form, with completed Accomplishments and Goals and Objectives sections, should be submitted to your supervisor (preferably electronically) at least two weeks prior to your Performance Appraisal discussion. In the Accomplishments and Goals and Objectives sections on the Performance Evaluation form, describe in detail outcomes achieved or anticipated. Include and group items as operational, strategic plan related, diversity related or for personal/professional development. For Goals and Objectives, be specific as to how success will be measured and include anticipated timeframe for completion. All stated goals/objectives should follow the SMART principle and be Specific, Measureable, Achievable, Realistic, and Timely. For more information including definition of the rating scale and the key performance criteria, please refer to the Performance Evaluation form. Please list Goals that were not completed, status of uncompleted goals, and what hindered your ability to complete the Goal:
