I. AUTHORITY 1. Per verbal instruction of Captain Mario S Braza NMSAI President / CEO 2. Coordination with Ms. Amy Delagente , DGSH Admin Manager II. MATTERS (TO BE) INVESTIGATED 3. to verify the alleged reported lost of two heads (2) game cocks 4. To establish when and how it occurred and to identify the suspect/s. 5. To find out if the incident is a case of an inside job. 6. To determine the liability of of the Security Personnel on the incident thru Negligence 7. To study the case and recommend security measures to prevent occurrence of same nature III- FACTS OF THE CASE 8. Report reached NMSAI thru phone call of Ms Amy Delagente -Admin Manager DGSH on or about 1000H 17 July 2011 informing the undersigned about the incident. 9. At the time of the investigation it was noted that the location where the game cocks were placed is not well lighted . 10, The guard on duty was stationed at the Main Entrance of the DGSH .monitoring the front area.

11. The area is almost two (2) hectares land area with inadequate lightings ,. 12. That the game cock were placed a few meters away from the dog house. 13. That no trace of footprints considering that the area is little bit muddy.
