RELEASE AND INDEMNIFICATION: I, the undersigned, represent the company, team or individual named above, and have full authority to authorize this release and indemnification. I hereby give and grant to the Habitat for Humanity San Bernardino Area (HFHSBA) the right to use, publish and copyright our dog house entry and photographs in promotion of the Dog House Building Competition. I agree that no advertisement or other material need be submitted to me for approval and HFHSBA shall be without liability to me for any distortion or illusionary effect resulting from the publication, portrait or likeness in any advertising and promotion of the HFHSBA. I hereby release and agree to indemnify the HFHSBA from and against any and all liability arising out of the exercise of the rights granted by this Release. Further, I hereby indemnify and hold harmless the HFHSBA from any and all claims arising from the display, use, transfer or ownership of our dog house entry. OWNERSHIP: The dog house becomes the property of HFHSBA once it is dropped off at the HFHSBA ReStore. The dog house must be delivered to the ReStore no later than 3:00 PM PT on March 16, 2013 to qualify for the silent auction of the HFHSBA Journey of Hope Gala, to be held on March 22, 2013. JUDGING: HFHSBA board members will judge the dog houses once they are delivered to the ReStore, the week before the Journey of Hope Gala. A prize will be awarded for the dog house that the Board feels is Best In Show. Judging will be at the discretion of the Board. Safety, creativity, and use of materials from the ReStore will be factors in voting. The winner will be announced at the Gala, and posted on the HFHSBA website. You need not be present to win. At least 3 dog houses must be entered in the competition; HFHSBA reserves the right to cancel the competition if fewer entries are received by February 16 th.

Entry Form must be submitted to the address listed above by 3:00 PM PT Saturday, February 16, 2013. There is no charge to register your entry. The dog house must be delivered to the ReStore between 9:00 AM PT on March 12, 2013 and 3:00 PM PT on Saturday, March 16, 2013. Be sure to provide a copy of at least one receipt that shows which item(s) were purchased at the ReStore and were used in the construction of the dog house. Official Rules Fact Sheet
Who We Are: Habitat for Humanity San Bernardino Area, Inc. (HFHSBA) is a faith based non-profit organization with the mission of eliminating poverty housing. We plan to achieve this mission by providing decent, affordable housing for low-income families by bringing together the necessary volunteers, experienced help, suppliers, city and government agencies, and families to form partnerships in an atmosphere of Gods love for all people. The organization is based on Christian principles and is open to everyone regardless of faith, including the Board of Directors, partner families and volunteers. Habitat is a tax-exempt 501(c)3 corporation and is included in group exemption number 8545. What We Do: Since the affiliate was incorporated in 1992, the organization has built or rehabbed 25 homes in Redlands, San Bernardino, Fontana, Loma Linda, Colton, Yucaipa and Highland, providing housing for more than 100 individuals. Competition Goals: To raise construction funds for Habitat for Humanity San Bernardino Area. All dog house entries will be auctioned off in a silent auction at our Journey of Hope Gala, to be held on March 22, 2013 at University Hall at the University of Redlands. Dog houses will be on display at the HFHSBA ReStore from when they are delivered to the ReStore until the day of the Journey of Hope Gala. Judging: HFHSBA board members will judge the dog houses once they are delivered to the ReStore, the week before the Journey of Hope Gala. A prize will be awarded for the dog house that the Board feels is Best In Show. Judging will be at the discretion of the Board. Sponsors Needed: If you or your business would like to be a sponsor for this competition. Please contact HFHSBA at info@habitatsb.org or (909) 478-1176. Where: Prior to the drop-off date at the ReStore, you are encouraged to display your completed dog house at your business or a business/organization. The dog house becomes the property of HFHSBA once it is dropped off at the HFHSBA ReStore between March 12 and March 16, 2013. The dog house must be delivered under manpower to the ReStore by 3:00 PM PT on March 16, 2013 to qualify for the silent auction of the HFHSBA Journey of Hope Gala, to be held on March 22, 2013.

Construction Rules
Measurements: Maximum Height: 4 feet. Maximum Width: 4 feet . Maximum Depth: 4.5 feet . Minimum Height: 2 foot. Minimum Width: 2 foot. Minimum Depth: 2 feet . Please account for the roof, overhangs, eaves or porch, chimneys, or other decorative items. If any measurement exceeds the maximum dimensions, the house cannot be accepted for entry or display. The house may be built in a way so that it can be easily assembled and reassembled by the auction winner. Materials: Dog houses must be built for outdoor use, using materials that are safe for dogs. No real fur or materials derived from animals are permitted. Dog houses must be moved into the ReStore by manpower. Please do all you can to keep the houses as lightweight as possible while maintaining a quality appearance. This is equally important once the owners take the houses to their homes, as most yards are not accessible to trucks or forklifts. Although creativity is expected and encouraged, HFHSBA reserves the right to deny delivery of any entry based on design content. The Journey of Hope is a rain or shine event. At least one item used in building the dog house must have been purchased at the HFHSBA ReStore. A copy of the receipt must be delivered at the time the dog house is delivered to the ReStore. Roofing: Please use materials that will withstand outdoor elements. Exposed Dangers: Please pay special attention to exposed corners and sharp edges. Please fasten any decorative items securely. Ensure any paints or sealers used are safe around pets. Make the house safe for any dog. Signage: If using signage, it maybe representative of your organization, team, or business; however, anything deemed inappropriate by the HFHSBA will not be accepted for entry or display. Transportation: The dog house must be delivered using manpower. Entry form: An Entry Form must be submitted to the ReStore by 3:00 PM PT Saturday, February 16, 2013. There is no charge to register your entry. The dog house must be delivered to the ReStore between 9:00 AM PT on March 12, 2013 and 3:00 PM PT on Saturday, March 16, 2013. Be sure to provide a copy of at least one receipt that shows which item(s) were purchased at the ReStore and were used in the construction of the dog house.
