The colored subheads/subtitles/column heads are the only informations not answerable by yes or no. The choices for columns C, E, G, I, K and L are found in the attached sheet. Write only the number(s) that represent(s) your reason(s). If your reason is not stipulated in the list of choices, please specify your reasons. 5. If a tick or a shade is inside Columns B, D and F, case or student is classified as red alert. The red shaded status means that the student-child needs attention and proper intervention to address his/her immediate needs. 6.
