The adviser or the guidance counselor can administer or conduct an interview to the student-child regarding the informations that are asked. 3. If your answer to the yes-no question is yes, kindly tick (/) or shade the box that corresponds to the question statement. If your answer is no, then leave the space or box empty. 4. The colored subheads/subtitles/column heads are the only informations not answerable by yes or no. The choices for columns C, E, G, I, K and L are found in the attached sheet. Write only the number(s) that represent(s) your reason(s). If your reason is not stipulated in the list of choices, please specify your reasons. 5. If a tick or a shade is inside Columns B, D and F, case or student is classified as red alert. The red shaded status means that the student-child needs attention and proper intervention to address his/her immediate needs. 6.
