NEW MIDWIFERY REQUIREMENTS

1 BASIC REQUIREMENT: 1.1 Original and photocopy of Transcript of Records with date of graduation, S.O. number, and scanned picture & with remarks: for board examination purposes only. CHED recognition Certificate for all new Schools/Programs; 1.2 Original & photocopy of birth certificate from NSO (SECPA). A photocopy of birth certificate from Local Civil Registrar is required if the NSO copy is not clear; 1.3 For married female applicants- original & photocopy of marriage certificate from NSO (SECPA). A photocopy of marriage certificate from the Local Civil Registrar is required if the NSO copy is not legible; 1.4 Three (3) copies of colored identical passport-size picture in white background and complete nametag; 1.5 Current CEDULA (CY2011); 1.6 Examination fee P 600.00; 1.7 Documentary stamps; 1.8 PRC window envelope with stamp ( for Baguio examinees only ) 2 ADDITIONAL DOCUMENTS: 2.1 Photocopies of valid Certificate of Registration (COR) and current Professional Identification Card (PIC) as a duly Registered Nurse (RN); 2.2. Accomplished Forms: a) Form No. 106 - twenty (20) Records of Actual Deliveries Handled) Form No. 107 five (5) Records of Actual Suturing of Perinea Lacerations) Form No. 107-A five (5) Records of Actual Intravenous Insertions Duly signed by the Registered Midwives (RM) who directly supervised the case requirements, and duly certified by any of the following officers/officials: 2.2.1. Municipal/City Provincial Health Officer (rural health unit, birthing, lying-in clinic, home delivery) and 2.2.2. Chief/Director of Hospital (hospital cases); 2.3. Certification of Consent/Approval, indicating that he/she had secured the consent/approval of the Director/Chief of Hospital or the Municipal Health Officer prior to his/her exposure to the concerned health facility.

3 ACTUAL HANDLING OF THE TWENTY-DELIVERY CASES/ REPAIR OF OBSTETRICPERINEAL LACERATIONS/ INTRAVENOUS FLUID INSERTIONS: 3.1. The procedures submitted shall be actually handled by the applicant after his/her registration as a Nurse. 3.2. The actual procedures shall be supervised by a Registered Midwife (RM) who has been entrained on the expanded functions as provided for under R.A. No. 7392. For midwives registered before 1996, or graduates under the old Midwifery curriculum, a copy of the Certificate of Training on the expanded functions shall be submitted. The Training Program prior to its adoption and implementation shall be approved by the CPE Council for Midwifery. This program can be obtained from accredited institutions/agencies such as the following: 3.2.1. Association of Philippine Schools of Midwifery, Inc. (APSOM) 3.2.2. Integrated Midwives Association of the Philippines, Inc. (IMAP) 3.2.3 Other Institutions/agencies currently approved/accredited CPE Providers by the CPE Council for Midwives created and supervised by the Professional Regulatory Board of Midwifery subject to approval by the Professional Regulation Commission. 3.3. The Registered Midwife (RM) who supervised the procedures shall provide the following information: 3.3.1. Photocopy of Employees ID, and 3.3.2. Photocopy of Professional Identification Card (PIC). 3.4. Actually handled cases shall not be more than one case a day to allow adequate time for the monitoring of labor, delivery, provision of postpartum care, and newborn care. 3.5. The Certifying Officer who is any of the Hospital Chief of Director, Provincial/City/Municipal Health officer, as the case maybe, shall provide the following: 3.5.1. Photocopy of Employees ID, and3.5.2. Photocopy of Professional Identification Card (PIC) Filing of application for the April 27-28, 2011 LICENSURE EXAMINATION FOR MIDWIVES will start on March 7, 2011. Deadline of filing is on April 7, 2011. How to apply for the exam: Educational Qualification / Training / Experience: Graduate Midwife Record of twenty (20) deliveries actually handled signed by Principal or Dean and duly notarized; 1996 graduates with record of five (5) sutures and five (5) intravenous injections.

REGISTERED NURSE PRC ID Card / TOR Record of twenty (20) deliveries actually handled with record of five (5) sutures and five (5) intravenous injections signed by Hospital Director or Municipal Health Officer and duly notarized. Scope of Examination . - The scope of examination for the practice of midwifery shall consist of the ff: a. Infant care and feeding; b. Obstetrical anatomy and physiology; c. Principles of bacteriology as applied to midwifery practice; d. Obstetrics; e. Midwifery procedures; f. Domiciliary midwifery; g. Community hygiene and first aid; h. Nutrition; i. Ethics of midwifery practice; j. Primary health care; k. Professional Growth and Development; l. Family planning; and m. Other subjects which the Board may deem necessary for addition or inclusion from time to time. Prerequisite and Qualifications of Applicants for Examination . - In order to be admitted to the midwifery examination, an applicant shall, at the time of filing of his/ her application therefore, establish to the satisfaction to the Board that he/she: (a) is in good health and of good moral character; and (b) is a graduate of midwifery in a government recognized and duly accredited institution. At the time of the issuance of a certificate of registration, the applicant shall be a citizen of the Philippines and at least eighteen (18) years of age. Schools of Midwifery . - To be recognized as a duly accredited and legally constituted institution for midwifery training as provided in this Act, a school of midwifery shall have a permit from the Department of Education,

Culture and Sports (DECS). The school shall have at least fifty (50) maternity beds and affiliated with an accredited hospital duly accredited by an authorized government agency or instrumentality. The school shall have a minimum ratio of one (1) clinical instructor to twelve (12) students in the hospital and in the community. Before allowing a student to graduate, the school shall satisfactory show to the Department of Education, Culture and Sports (DECS) or other authorized government agencies that the student has completed the course in midwifery as prescribed by the DECS or other duly authorized government agencies, and that during the training, the student has personally attended at least a total of twenty (20) deliveries in the hospital and in its domiciliary services. Qualification of Faculty . - The faculty shall have academic preparation appropriate to teaching assignment, as follows: (a) At least a bachelors degree holder or doctor of Medicine (M.D.); (b) Proof of competence in the field of specialization assigned to him; and (c) A registered nurse or registered midwife in the Philippines with at least one (1) year of satisfactory teaching experience or one (1) year of efficient performance in maternity ward/ community: Provided, That a registered midwife may be allowed to follow-up student midwives in the community ward provided she has at least two (2) years of experience in the area of assignment and has undergone training in the supervision of students.
