MEMORANDUM FOR ARMY FAMILY HOUSING RESIDENT

FROM: RCI AND HOUSING DIVISION, FORT WAINWRIGHT, ALASKA

SUBJECT: Privacy Act Release

1. North Haven Communities LLC (NHC) is the private company that will soon own and
maintain the home you are living in. Your voluntary signature on this letter is requested to
authorize the Army to provide your personal information currently maintained by the
Housing Services Office (HSO) to NHC. This information may include:

a. Soldier's name, Grade and SSN

b. Housing application, travel orders and command sponsorship approval
c. Family member names and birthdates
d. Home and duty phone numbers
e. Complaints, reprimands, eviction notices, yard violations
f. Birth certificates, marriage certificates, adoption documents, custody documents,
divorce decree(s)
g. Home based business authorization or disapproval
h. Any information available on pets
I. Any and all documents pertaining to the assignment of your housing, including
but not limited to, inspections, deficiency lists, work orders, health and welfare
reports, show cause and requests or exceptions to policy requests/decisions

2. Failure to sign this letter authorizing the transfer of your privacy act information will
result in you having to provide similar or duplicate information to NHC. Some or all of this
information may also be in electronic form at or in an Army approved database. All of these
records contain information useful to NHC in serving your needs as a resident of Fort
Wainwright Privatized Housing.

3. You may access up-to-date information on North Haven Communities progress through
the transition to privatized housing, by visiting their web site at www.nhcalaska.com or by
contacting your future community manager at 356-1882 for the North Post (Siku Basin,
North Town, Chena Bend and Denali Village) or the South Post (Southern Cross, Northern
Lights, Gerstch Heights, Bear Paw and Taku Gardens) at 356-1883. You may also visit their
office inside of the Welcome Center at Building 3401 Santiago Road where they are co­
located with the Housing Services Office or contact them at 356-1881.

Office, by fax (907-361-4508), scan and email to (connie.kiser@wainwright.army.mil), or
