I am aware that the Abu Dhabi Scorpions Ice Hockey Club, further mentioned as ADSIHC, including but not limited to all players and officials of the Abu Dhabi Hockey League (ADHL) is a non-profit organization and its purpose is to provide competitive, recreational ice hockey, further mentioned as hockey, for all level of players. I am conscious that while the ADHL is a non-checking league, hockey is a fast moving physical game and but that body contact is an inevitable part of the game. I am aware that it is my responsibility to wear all protective equipment required for the sport of hockey and that all such protective equipment must be duly certified. Helmets other than hockey helmets or playing without a helmet are prohibited. I am aware that playing hockey in the ADHL involves many risks, dangers and hazards including, but not limited to, impact and collision with other players and/or officials, impact with objects or equipment included but not limited to hockey sticks, skates, boards, etc., loss of balance, and negligence of other players and/or officials during games. I am aware the ADSIHC and ice rink staff cannot be held accountable for broken items or equipment or any theft of personal items including but not limited to mobile phones, wallets, hockey bags, jewelry etc., incidents while using the bathroom facilities, or while anywhere in, or around, the rink. I am aware of all risks, dangers and hazards associate with playing in the ADHL, I freely accept and fully assume all such risks, dangers, and hazards and the possibility of personal injury, death, property damage and loss resulting therefrom. I am aware that the ADSIHC will not provide any kind of personal insurance. All medical attention as a result of an accident or incident while playing hockey or while linger in the ice rink, as well as driving to or leaving the ice rink or while linger outside the rink, especially the parking lot, has to be covered by myself. I am aware that it is my responsibility to contact my healthcare insurance company to assure medical aid is provided and covered for accidents arising while playing hockey and I am aware may have to pay for extra costs to cover dangerous sports liability. The ADSIHC will not provide financial benefits in this matter. I am aware that the Emirates Hockey League (EHL) is not a part of the ADSIHC and therefore that the EHL Scorpions are not a subdivision of the ADSIHC. I am aware that the ADSIHC is not responsible and accountable of my participation or involvement in the EHL of any sort including but not limited to medical expenses, claims of any sort, death, injuries etc. and that I freely accept and fully assume all such risks, dangers, and hazards. I am aware that the ADSIHC is not responsible and accountable for my participation or involvement in hockey tournaments with teams formed from ADHL: players, including, but not limited to, medical expenses, claims of any sort, death, injuries etc. and that I freely accept and fully assume all such risks, dangers, and hazards. I am aware that I am a visitor in a Muslim country and therefore have to obey the rules and regulations of the host nation. Breaking of the laws of the UAE includes but is not limited to consumption of alcohol and illegal drugs, foul language, libel of others, sexual harassment etc. I am aware that the ADSIHC will address the police if I am using illegal drugs. I am aware that consuming alcohol while on the property of the Zayed Sport City, further mentioned as ZSC, is only allowed within licensed establishments and that the consumption of alcohol anywhere else on the property of the ZSC is not allowed by UAE law. I am aware that the consequences of breaking this law may result in jail time, fines or deportation and that I am fully responsible. I am aware that the ADSIHC officials are obligated to deny me access onto the ice if there is proof of intoxication of any kind. I am aware that I will not be reimbursed, financially or of any other nature, for the loss of ice time including but not limited to denial of ice time due to intoxication, penalty minutes and game penalties, vacation on my behalf, public holidays, ice rink closures, or any other nature.

RELEASE OF LIABILITY, WAIVER OF CLAIMS (Please print your name on the line under paragraph A) and initial paragraphs B) and C)).
A) I,_______________________________________, hereby waive any and all claims and hold harmless and indemnify the ADSIHC, the ice rink staff, and the Abu Dhabi Ice Sport Club (ADISC) including all affiliated parties, from any claims that I or my kin have or may have in the future, including all liabilities for any loss, damage, expense or injury including but not limited to death or any kind of paralysis that I may suffer or my next of kin may suffer resulting from my involvement and participation in the ADSIHC and the ADHL. This Agreement shall be effective and binding upon my heirs, next of kin, and representatives, in the event of my death or incapacity.

C) This Agreement and any rights, duties and obligations of the parties to this agreement shall be governed by and interpreted solely in accordance with the laws of the United Arab Emirates in correlation with the laws and regulations of the Emirate of Abu Dhabi and no other jurisdiction; and any litigation involving the parties to this agreement shall be brought solely within the Emirate of Abu Dhabi and shall be within the exclusive jurisdiction of the Courts of the Emirate of Abu Dhabi.

I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, AND REPRESENTATIVES MAY HAVE AGAINST THE ADSIHC AND ITS SUBDIVISIONS AND AFFILIATED PARTNERS INCLUDING BUT NOT LIMITED TO THE ADHL, ADISC ETC. I AM AWARE THAT I CANNOT PARTICIPATE OR BE INVOLVED AS A PART OF THE ADSIHC AND THEREFORE CANNOT PLAY OR STEP ON THE ICE DURING OFFICIAL ADSIHC TIMINGS IF THIS AGREEMENT HAS NOT BEEN SIGNED BY ME AND A WITNESS. I AM AWARE THAT BY SIGNING THIS AGREEMENT I WILL OBEY THE RULES AND REGULATIONS OF THE ADSIHC.
