Questions 1-6 are based on the following scenario St. Barnabas is a 15 year old type IV health centre situated on one of the arterial roots of an urban center. In its first 5 years it enjoyed a solid reputation for clinical care, provided for a small user fee. Dr. Thompson an experienced public health specialist was its first manager. He knew alll the staff members whether they were professional technical or auxilliary and had a reputation of being firm but fair. He retired after running the facility for its first 5 years. Towards the end of his tenure it was noted that the clinic was beginning to attract a larger number of patients than before. This was presumed to be due to its excellent reputation. In the past 10 years several events have occurred. Three successive administrators have been appointed. One a physician, the two others trained health administrators. In the last year the new government has abolished user fees. The last administrator had done a situational analysis which was so impressive he had been invited to apply for the post. He however resigned after six months stating that he had been given basket to carry water. Over the last few years there has been an exodus of all categories of staff. Many nurses have migrated and an equal number retired. So that the nursing establishment was at 60% capacity. The pharmacists (three) were very short, there place being taken by technicians. More recently there have been flair-ups between staff and patients which have made their way into the newspapers. Doctor Goodheart is now the Physician in charge. Relating to an administrator who in turn reports to the regional health authority. The day after Dr. goodheat takes over he calls a general staff meeting and aks for ideas to tackle the clinic problems. One of the original staff members becomes aggitated and says what we need is someone with the style of Dr. Thompson. There was order when he was here. Yes says another he could command and control. This sense of motivation is further enhanced when there is team or public recognition of the achievement. Sometimes there is conflict between individual and organizational goals. For example, seeing patients thoroughly, versus seeing them quickly might create conflict in physicians. Some relevant issues. 1. These conflicts are more evident now that health care services are operated more using business and management principles. 2. Professionals often want to work with state of the art equipment and medications, both for personal satisfaction and better patient care. The organization may not be able to provide 'cutting edge' technology or medication. 3. Limitations, in respect of senior positions in the organization are also frequent sources of frustration. Let us now look at some of the ways that frustration manifests itself in individuals at the work place. Frustration leads to stress, conflict, anxiety, and burnout. The most mature response to frustration is to engage in:

problem solving, individually or collectively.

Problem solving should begin with a diagnosis of the situation. This will include those factors coming from 'problems within the system'. These might be related to how work is organized, and who does what, some members of the team may be overburdened, while others may be underutilized. Alternatively or additionally, there may be an attitudinal problem preventing the individual or organization from attaining the goal. To effect a solution, though the diagnosis and possible solution have to be shared with those having the power to effect change, the problems and the solutions have to be aired at the correct forum and at a point when individuals are felt to be ready to listen.
