ADMISSION POLICY FOR GRADE SCHOOL School Year 2013-2014


A. ADMISSION 1. All applicants to Xavier Nuvali Grade School are required to undergo the admissions screening process. The screening process provides the school with data to determine acceptance or nonacceptance of an applicant. 2. Parents will be informed about the result of the screening process. However, the details of the results will remain confidential. 3. The applicant is given the admission test once only. Re-testing is not allowed. 4. All applicants should have passing grades in all subjects from previous grade level and in the First Quarter of the current grade level. B. GENERAL NORMS 1. A candidate can succeed in meeting the requirements of Xavier School Nuvalis Grade School Program if he has attained satisfactory evaluation in the following areas: a. Attention Span b. Interpersonal Skills c. Communication Skills in English comprehension expression d. Competence e. Positive self-discipline f. Stress Tolerance g. Respect for Members of the School Community 2. The school also takes the following into account: a. the willingness of the parents of the student applicant to collaborate actively with the school in the education and formation of the student (which includes the parents acceptance of the school policies, especially those found in the student handbook); b. the willingness of the applicant and his parents to be involved in important school activities; c. the extent to which the applicants family environment is open to, and supportive of the Catholic Christian values espoused by the school. C. QUALIFICATIONS FOR ADMISSION Passing Grade School Entrance Examination (GSEE) Results Performance In The Current Grade/Year Level An applicant with a failing average by the end of March 2013 will NOT BE ACCEPTED. The following shall be taken into account: Composite score of the applicants previous grades (4 quarters) First quarter grade of the current level IQ score 3. Disciplinary Record An applicant who has been under any major disciplinary status or involved in any major disciplinary case within the last school year will NOT BE ACCEPTED.

D. REQUIRED DOCUMENTS

1. Letter of Intent to Transfer 2. Birth Certificate from NSO or Certified True Copy (original & photocopy) 3. Baptismal Certificate if baptized in the Roman Catholic Church (original & photocopy) 4. Vision and Hearing Screening Test Results 5. Duly Accomplished Application Forms 6. Duly Accomplished Health Record Form 7. Duly Accomplished Recommendation Form from Current School 8. Original copy of the report card in the previous grade level 9. Photocopy of the grades in the First Quarter of the current level 10. Three (3) copies of recent identical 1.5 x 1.5 photos in white background (scanned &
computerized pictures not allowed) to be pasted on the following: Application Form Health Record Form Test Permit 11. Two (2) copies of recent identical family picture in size 4R (scanned & computerized pictures not allowed) to be pasted on the Application Form and Student Information Sheet. (Only immediate family members should be in the picture.) Additional Requirements for Naturalized Filipino and Non-Filipino Applicants: FILIPINO NON-FILIPINO (PERMANENT RESIDENT) 1. ACR: Alien Certificate of Registration (photocopy) NON-FILIPINO (TEMPORARY RESIDENT) 1. ACR: Alien Certificate of Registration (photocopy) 2. Immigrant Certificate of Residence (photocopy) 3. Study Permit 4. PASSPORT pages (photocopy) showing the following info: Passport number Place issued Date issued Expiration date Date of entry into the Phil. Entry status Length of authorized stay

F. APPLICATION PROCEDURE APPLICATION CAN BE DONE EITHER IN XAVIER SCHOOL, SAN JUAN CITY OR XAVIER SCHOOL NUVALI. PLEASE FOLLOW THE FOLLOWING STEPS: 1. Present the following pre-application requirements to the Registrars Office: a. Letter of Intent b. NSO Copy of Birth Certificate ( original copy) c. photocopy of First Quarter Evaluation of Performance in the current level d. original copy of Report Card in the previous level (for all levels applicants, except Kinder) 2. Get a Payment Slip for the Application and Testing fee from the Registrars Office. 3. Pay the non-refundable Application Fee of P500.00 in cash to the Accounting Office. 4. Present the receipt of payment and claim the Application Kit. 5. Submit the following to the Registrars Office for checking: a. Duly accomplished Application Form b. Duly accomplished Health Record Form c. Duly accomplished Recommendation Form d. Vision & Hearing Screening Test Results e. All Other Required Documents 6. Upon completion of the required documents, you will be issued a Test Schedule and Test Permit. 7. Report to the Testing and Research Center on the scheduled testing date at least ten (10) minutes before the scheduled testing session. Be sure to bring the test permit. NO TEST PERMIT, NO ENTRANCE EXAMINATION. G. APPLICATION RESULTS 1. Results will be released by batches depending on the applicants test schedule. 2. No results will be released by telephone. 3. Results of application will have to be picked up at the Registrars Office (Xavier School San Juan) or at the Principals Office (Xavier School Nuvali). They will not be sent to you. 4. Parents or their representatives may get the results. Representatives are required to present an Authorization Letter and an ID for proper identification. 5. If the results are not picked up 4 days after the release date of the test results, we will assume that you are no longer interested in your childs admission to Xavier School Nuvali. His/Her application will be automatically cancelled.

H. ENROLLMENT RESERVATION FOR ACCEPTED APPLICANTS The parents of applicants who have been accepted are required to:

1. Make a reservation to express their intention to enrol their child in Xavier School Nuvali.
N.B. The acceptance letter will include instructions on how to make this reservation and to pay the required Reservation Fee. 2. Submit photocopy of parents marriage certificate if applicable.
