Social Worker (MSW)

Under the direction and supervision of the Center Manager and the Social Work Specialist, the Social Worker is responsible for providing direct casework services to On Lok participants. DUTIES / RESPONSIBILITIES: Practice clinical social work by establishing and maintaining a therapeutic relationship with a caseload of participants and their families/support systems. The Social Worker will be able to perform thorough initial psychosocial assessment and ongoing required evaluations, identify any warning signs and other problems, set measurable objectives and develop and implement appropriate and achievable individualized care plan, such as providing individual and family counseling. The Social Worker will also adhere to all ethical and legal standards of social work practice, such as confidentiality and mandated reporting criteria. Being a Case Manager, the Social Worker acts as the primary liaison between participants, families, team and the agency through effective communication. The Social Worker will initiate, coordinate and mediate care conferences with participants, families and team members for purposes such as education, updates on participants condition, clarification of care plan and support. The Social Worker will also identify service needs for each participant, advocate, coordinate and follow up with both internal disciplines and outside resources. Fulfills all documentation requirements in a timely and accurate manner. Maintains progress record for each participant in ICCIS according to the individual care plan or at least once a quarter. Enter progress notes for acute circumstances within same day of service. Respond to monthly incomplete log within one week from date of issue. Complete initial and re-evaluation within required timeline. Participate actively in team discussions. Advocate on behalf of the participants and be able to compromise differences among team members to support team plan. Other related duties as assigned. QUALIFICATIONS (knowledge, skills, abilities) Graduate of an accredited college with a MSW degree. Minimum one year experience in the geriatric field preferred. Interest, demonstrated ability and/or experience in working with the elderly. Ability to work independently and as part of multidisciplinary team. Highly organized and has the ability to prioritize and work in crises. Maintains a flexible and creative attitude. Computer literate.
