AFIC Waiver

RELEASE OF

LIABILITY I hereby agree to participate in the summer

hockey sessions at my own risk and peril. I understand that Appleton Family Ice Center (AFIC), its directors and officers, all coaches, assistant coaches, AFIC management, facility owners, and operators, its associates, licensees, employees, agents and/or representatives will not be held responsible for death or any accident, injury, loss or damage however caused. I further agree to release and hold them harmless from all claims, damages, actions, loss, expense and demands which may arise as a result of, or by reason of death, injury, loss, damage or medical expense may have been contributed or occasioned by the action, inaction or negligence of any or all of them. Notwithstanding anything to the contrary, if I am successful in any suit, action, or proceedings against them for any damages, loss, cost or expense (including punitive and/or exemplary damages) the award (including court costs and fees) shall be limited to the amount of registration fee paid by me for this program. I understand that I will provide my own equipment. This release shall be binding on the players heirs, assigns, executors and administrators.
