C. Home and Environment The Calderon Familys House is made of concrete and has one bedrooms, and a kitchen which is not enough or adequate for their living space. Every member of the family sleep in one bedroom. Mr. and Mrs. Calderon share bedrooms with their siblings which is not enough for them to have a rest and sleep and other family member sleep in the living room at night. The Laundry area outside the house, kitchen sink cabinet and the garbage cans are the resting sites of vectors of disease such as flies, mosquitoes and roaches present in Calderon familys house. Their foods are stored in the table with a basket and a cover. The Calderons water supply comes from NAWASA. The water coming from the NAWASA is used for washing, cleaning, and bathing and for drinking purposes. The Calderon family doesn`t have a bathroom and toilet but their only using is the public bathroom for all who lives in their place. Their garbage is taken out everyday and collected by garbage trucks everyday. They have one garbage cans outside the house. D. Assessment prior to introduction

The area is congested, there are families in a compound who is expose to several health hazards like poor environmental sanitation, open drainage system and inadequate living space. The Calderon family is one of those families living in the area. Mrs. Kita is busy taking good care of her child who is about to sleep at the time when I arrived for the initial interview. Objectives

The interview took placein front of Mrs. kitas house at exactly 10:10 a.m. It lasted for about 15 minutes and I was very glad that Mrs. kita was very accommodating. I did not hesitate to ask relevant questions because she showed a lot of interest in the conversation. Brief description of the setting

I was able to visit Mrs. kitas place twice and somehow I noticed that their house has no enough lighting and ventilation. I wonder how they manage to stay there for two years without water supply and no private toilet facility. Their house is considered to be a make shift type of house and has inadequate living space. The wooden bed found near the main door serves as the living room area leaving a small space for kitchen Evaluation

Based on my interview, the family needs more orientation and information regarding health issues. They should value or prioritize their health above all. The importance of prenatal check up and malnutrition should be given emphasis to ensure the development of the baby. Inadequate living space and poor environmental sanitation seems to be a threat to the familys health. Overall, the

health requirements of the family are not being met due to poverty and lack of awareness. Future Plan or Objectives for the next interaction

Promotion of health, prevention of diseases together with simple treatment and rehabilitation would be my next objective to help the family alleviate their health status and conditions

FAMILY NURSING CARE PLAN Problem Identified: Inadequate Immunization Status of the Child Family Nursing Diagnosis: Inability to recognize the presence of health threat due to lack of knowledge about the condition Goal of Care: Within 4 hours of nursing interventions, the family will be able to determine the importance of having complete immunization. Objectives: After two home visits, the family will be able to: 1. determine the importance o complete immunization of children; 2. enumerate the possible illnesses that can occur due to incomplete vaccination; 3. follow-up the vaccine of the children; 4. give specific attention to the schedules of the childrens immunization; 5. understand the advantages of having completion of the immunization.

Goal of Care: Within 4 hours of nursing interventions, the family will be able to determine the importance of practicing proper methods on waste disposal. Objectives: After two home visits, the family will be able to: 1. identify the different ways on proper disposal of garbage such as: a. use of compost pit with cover; b. segregate the non-biodegradable and biodegradable materials; c. recycling of can-be-used garbage; d. reusing or selling of some garbage like cans, bottles and plastics. 2. enumerate the proper techniques on keeping the surroundings clean and through using proper method of waste disposal; 3. define the meaning of proper garbage disposal and its advantages; 4. recognize the possible effects of garbage burning; 5. verbalize understanding about the importance of practicing proper waste disposal
