    1. Title page of the report (Title, Name of Organization, Author, Purpose and Date) 2. Preliminary pages (i) Declaration (ii) Acknowledgement (iii) Short summary or abstract (iv) Table of contents 3. Chapter one - Background Introduction (one page maximum) Profile of the organization you worked with or project you worked on (5 pages maximum) 4. Chapter two Field attachment activities (5 pages maximum) 5. Chapter three Emerging issues / discussion of experiences (5 pages maximum) 6. Chapter four Conclusion and Recommendations (2 pages maximum) 7. References (if any) 8. Appendices (E.g. applications you were exposed to, working documents you produced, work plan, working tools, any codes written if programming was involved e.t.c)
