CARMENCITA CRISOSTOMO, and a resident of Brgy. Alma Villa, Gloria,
Oriental Mindoro Philippines, after being sworn in accordance with law, depose
and state:

That I am the owner of a CHAIN SAW machine described as follows;

Brand: STIHL
Serial: 202-02-1958
Date of Acquisition: April 11, 2011

That I acquired a Brand New unit of CHAIN SAW from CALORMIN

ENTERPRISES on April 11, 2011 with Sales Invoice number 4670

That I used the said chain saw in my legitimate business of buy and sell,
and contracting and cutting of lumber and related products;

That as owner thereof, I undertake to obey and follow the laws and
pertinent local resolutions and ordinances that may be imposed or serve as
limitations to the use of the said chainsaw;

I also declare that I have never been charged of any offense that may
have relation to or connection with violation of Presidential Decree 705 or
other related laws;

I am executing this statement to attest to the truth of all the foregoing

atatements.
