I Mr. Guggilam Kalyan Butchi Rama Reddy alias G.K.B.R. Reddy son
of Shri Guggilam Bangara Raju do hereby solemnize and affair on oath that.

1. I am an Indian Citizen by birth born on 20th April 1982 at

Parvathipuram, Vizianagaram District, Andhra Pradesh.

2. That at present I am presently residing at 402, 9A, Siddheshwar
Tower, Sector-19, Kamothe, Khandeshwar, Navi Mumbai – 410209.
3. That I have changed my name from G.K.B.R. Reddy to Kalhyaan B

Gugilam due to numerology. I have published it in the gazette of

State of Maharashtra viz. registration no. U-26938 under page no.75
4. That in my Graduation Certificate / Date of birth Certificate my name

is Guggilam Kalyan Butchi Rama Reddy alias G.K.B.R. Reddy.

5. That in the month of January 2010 my name has been changed from
G.K.B.R. Reddy to Kalhyaan B Gugilam and in my all future
correspondence I will use my changed name i.e. Kalhyaan B Gugilam.
