I, MARIA MELREN M. LEONADO, of legal age, Filipina, single, and a resident of

1055 Maceda St. Sampaloc, Manila, after having duly sworn to in accordance with law, do
hereby depose and states THAT:

1. I am the daughter of the late RENATO C. LEONADO;

2. Died on February 25,2011 at Lopez, Quezon;
3. That the following are lists of the funeral expenses for the burial of my father;
