WEB Publishing Permission Form

Hampden Academy publishes web pages that may be viewed on the world-
wide web. Your child’s school has a web site which is a part of this activity.
The objectives for publishing these pages include sharing, communicating,
and providing information and resources to our students and staff.

It is Maine State Law to seek parental permission before including your

child’s photo, first name or creative work on the Internet. All creative work
will be copyright protected.
