    Go to http://www.phmhealth.com and click on Policy & Claim Status in the Utilities menu on
    the left hand side of the homepage.

    Once on the Policy and Claim status page follow the steps mentioned below:
    Step 1: Select Insurance Company – HDFC ERGO GENERAL INSURANCE COMPANY
    Step 2: Search by – Select Employee ID
    Step 3: Enter Group Code – FSF
    Step 4: Enter Employee ID – Enter your Employee ID
    Step 5: Enter Password – Enter your “Date of Birth” in format (DD/MM/YYYY)/You’re
    Changed password
    Step 6: Then click Submit

    You will find the below screen
