2010 PHOENIX AWARD NOMINATION FORM

The P.E.A.C.E. Initiative (Putting an End to Abuse through Community Efforts) is a coalition of public and private institutions,
grassroots organizations and individuals working collaboratively to end family violence. This group works diligently to raise
community awareness about the dynamics of domestic violence, the extent of the problem, resources available and safety planning.
The P.E.A.C.E. Initiative will recognize this October as National Domestic Violence Awareness month. As one of its awareness
projects, the P.E.A.C.E. Initiative will present the twelfth annual Phoenix Awards. The Phoenix, a mythical bird, when it feels its
life is at an end, builds a nest, sets it on fire and is consumed by the flames. From the ashes, a new Phoenix springs forth to live
again. Like the Phoenix, the abused woman must rise from the ashes of an abusive relationship to find life again.
Help us recognize those unsung individuals within our community who have endeavored to work to change the consuming effects

The Phoenix Awards will be presented in a ceremony to be held on Tuesday, October 26, 2010, 6:00 pm at the Mexican American
Catholic College, 3115 W. Ashby Place.
