Note: The training reports shall be submitted to the HR Assistant for Staffing. The
trainee’s trainer/supervisor acknowledges completion by affixing his signature on the
cover page of the report. Certificate of completion shall be issued by HR upon
completion of mid and post training reports and other company requirements pertaining
to the On-the-Job training.
