I Pravitha Santhosh aged 30 years wife of Sri. Santhosh, Vattanal

Puthenpurayil House, Thirumarady Village, Muvattupuzha Taluk, Ernakulam District
do hereby solemnly affirm and state as follows:

My eldest daughter has been named as MALU SANTHOSH in the Birth

Certificate issued from the Births & Deaths Office at Muvattupuzha as per certificate
dated 3..2..2006

I solemnly affirm and state that the above name MALU SANTHOSH may be
changed as NANDANA SANTHOSH. Her Date of Birth is 20.12.2005.

Again I may state that the reason to change her name is as follows. In the
Ration Card, her name has been shown as NANDANA.

Further I may state that she has not been admitted in any school till date.

I request that due change may be noted in the register and a fresh certificate
may kindly be issued at the earliest.

Whatever I have stated above is truth and nothing but truth.
