freely and voluntarily depose and state:
That we are the parents of ALCIDES LUCAS I. CHUNACO, born on July
07, 2007, at Manila;
That our son is scheduled to travel with his godfather, ATTY. ALBERTO
PEDRO B. ARCILLA, to Dubai, United Arab Emirates on April 23 to 27,
2015;
That we have sufficient and adequate financial capability to defray all
his expenses during and for the duration of his travel abroad with his
godfather;
That our son will not become a public charge that would embarrass us
and/or the Republic of the Philippines;
That this affidavit is being executed for the purpose of informing the
proper authorities of our full consent for our son to travel abroad with
his godfather.
