I, JOHNHENRY YU DIET BOJA, Filipino, of legal age, and residing at 2553 Mabini
Ext Magupo Poblacion, Tagum City, Davao, after having been sworn in accordance
with law hereby depose and state:
1. That last 18 June 2013, I bought a motor vehicle from Florentino

Giron Arcilla Jr., which said motor vehicle, subject of our deed of
sale executed thereof, was still registered under the name of
Marcelito G. Mercado, whom the former had first acquired the
same, which is more particularly described as follows:

2. That prior to the acquisition of the above vehicle, the same has

been previously mortgaged from the UNION BANK (previously

mortgaged from THE INTERNATIONAL CORPORATE BANK) that was
transacted by Mr. Mercado.
3. That the principal loan has been fully paid, and the bank was issued

a cancellation of mortgage, releasing the above mentioned vehicle,

however, the copy of the copy of the certificate of cancellation of
mortgage of the above vehicle that was issued by the bank has
been lost, or might have been misplaced in unknown circumstances
and despite exerted diligent efforts have made to locate the same,
but to no avail.
4. As such, I am executing this affidavit to attest to the truth of the

foregoing instrument and for any legal purposes this may serve.
