2) My compensation is on a per trip basis only;
3) My main responsibility is to carefully deliver all the cargo enclosed in the truck I am
assigned, to its respective destinations, free from neither breakage nor damage;
4) I undertake to carry the responsibilities given to me with care and proper diligence all
the time;

If by chance, an untoward and unexpected incident happens leading to the

deterioration and damage of whatever goods or cargo have been assigned to me to
deliver, I likewise agree to share the responsibility of compensating the personality
whose goods and cargo are damaged with my employer;

6) I, together with my employer, will tale account to all the damages incurred from any
untoward ad unexpected incident that will arise out of my capacity;
7) I further undertake that I will not evade with any of my responsibilities arising from this
Affidavit;
8) I am executing this to attest the truth of the foregoing, and for whatever legal purpose/s
it may serve.
