The Greenport Skate Park Festival is accepting applications for vendors at its 2nd Annual Skate Park Festival, to be held on
Saturday July 17, 2010. 10:30 AM - 8 PM. Rain Date Sunday July 18, 2010
One day fee
• 10'x10' Tables/Tents $50.00 (non-refundable)
• Trucks $75.00 (non-refundable)
• Non-Profit organizations: NO CHARGE

1) Please provide a brief description and/or photographs, website, jpegs, myspace etc of items you will display.
Vendors can set-up on Sat July 17 at 9A M - 11 AM
New York State requires that we have your Taxpayer ID. # or social security # on file. Please include this information on
your application.

2) Complete Release.

RELEASE and ACCEPTANCE

General Release and Acceptance of Rules - This Contract is for the Greenport Skate Park Festival to which vendor applies
or is accepted, I, the applicant, have read the “Conditions of the Festival" of this contract and agree to abide by said
conditions. In addition, I, the applicant do expressly release the Greenport Skate Park Festival & Greenport Village of and
from any and all liability for any damage, injury or loss to any person, business or property which may arise from the
licensing and occupation of the vendor space by the applicant, and agree to hold the Festival harmless of any damage, injury
or loss by reason thereof.

I understand that if this contract is not accepted, Vendor Fees will be returned by mail. If this contract is accepted, I give
permission to use my name, business name, photographs, videotape, or images taken of me or any items for any and all
purposes. The one day fee is non-refundable.
