having duly sworn to in accordance with law, do hereby depose and say;
That I am a bona fide member of Social Security System (SSS) with SSS
NUMBER 0608529271;
That I engaged formerly as an street ambulant vendor, located at
Commonwealth, Aurora, Zamboanga del Sur;
That due to old age, I ceased to be as an street ambulant vendor on
March 31, 2016 and since then, I have no income;
That I am executing this affidavit to attest to the truth of the forgoing
statements that I have no income since March 31, 2015 and any legal
purpose this may serve;
