AFFIDAVIT
I, MURNA ARUGAY BANGAYAN, of legal age, Filipino and a
resident of Cataggaman Viejo, Tuguegarao City, Cagayan, after having
been duly sworn to in accordance with law do hereby depose and state
that:
1.
When I secured a copy of my Certificate of Live Birth from
the National Statistics Office (NSO), Tuguegarao City, Cagayan, I
noticed that my name was erroneously written as MURNA ARUGAY,
contrary to what I have been using in all my scholastic records and
other important documents as MYRNA MALANA ARUGAY;
2.
It should be made clear that my name MURNA MALANA
ARUGAY in my Certificate of Live Birth with Registry No. 1277 refers to
the same person of MYRNA MALANA ARUGAY;
3. I am executing this affidavit for the purpose of my change of
name with the City Local Civil Registry Tuguegarao City and for all such
other legal purposes that this affidavit may serve.
IN WITNESS WHEREOF, we have hereunto set our hands, this
