Speech-Language Pathology Evaluation Report


Joseph Kiger, a twenty four-year old female, was seen for a speech and language evaluation at
the Louisiana State University Health Sciences Center (LSUHSC) Speech-Language and Hearing Clinic
on June 22 and June 24, 2015 due to concerns regarding academic performance. The following
information was obtained from the case history, personal interview, formal evaluation results, and
informal observations obtained at the time of the evaluation.

Background Information
Joseph attended the evaluation alone and reported the following information during an interview
conducted at that time. She described a history of academic difficulty and challenges reading. At age 11,
an educational psychologist diagnosed her with dyslexia. Joseph reported that she received different
color lenses for reading and it was suggested that she sit at the front of the class. She reported that the
lenses did not help her with reading and she used them for a short time. Joseph did not receive academic
accommodations in high school or in college, where she received a Bachelor of Arts in Speech and
Hearing Sciences. At the time of this evaluation, Joseph was enrolled as a second year speech-language

pathology masters student. Recent academic difficulties have prompted a more in depth examination of
Joseph s language skills.
Language
The Oral and Written Language Scales II (OWLS II) was administered to assess Joseph s
receptive language (comprehension), expressive language (speaking) skills. Linguistic structures
measured in each section included lexical/semantic (i.e. vocabulary), supralinguistics (i.e. nonliteral
language, such as inferences and figurative language), and pragmatics (i.e. functional and social
language). The OWLS II is normed through 21 years of age; however, Joseph s results are believed to
be indicative of her language skills. The following percentile ranks are based on a chronological age of 21
years.

The Listening Comprehension scale was obtained by the examiner presenting a verbal stimulus.
Joseph selected one picture out of four that best depicted the meaning of the verbal stimulus (i.e. show
me which pencil is the shortest). Joseph received a standard score of 121, which correlated to a
percentile rank of 79. This indicated average receptive language skills.
The Oral Expression scale was obtained by the examiner asking a question or reading an
incomplete sentence regarding one or more pictures. Joseph was asked to answer the question or
complete the statement correctly. For example, In order to meet his many upcoming deadlines, Steve
work around the _____. Joseph received a standard score of 122, which correlated to a percentile rank
of 86. This indicated average expressive language skills.
The Listening Comprehension and Oral Expression scores were then combined to determine an
Oral Language Composite. This score represents Joseph s overall receptive and expressive language
ability. She obtained a standard score of 118, indicating average oral language skills.

Academic Skills
The Woodcock-Johnson IV Test of Achievement (WJ IV ACH) was administered to assess
reading, oral language, written language, mathematical abilities, and academic fluency. The WJ IV ACH
has a mean of 100 with a standard deviation of +15. Standard scores of 90 110 and percentile ranks of
25 75 are considered average. Joseph obtained the following results:

The subtests were analyzed in clusters for achievement strengths and weaknesses and for
comparison with age level peers. Subtests are described followed by clusters.
Letter-Word Identification measured Joseph s ability to identify letters and words. She was not
required to know the meaning of words. Joseph received a standard score of 75 and a percentile rank of
5 indicating scores in the low range. Low performance on this subtest may be a function of inefficient
strategies for word identification. Joseph may not have developed automatic word identification skills.
Applied Problems assessed Joseph s ability to analyze and solve math problems. To solve the
problems, she was required to listen to the problem, recognize the procedure to be followed, and then
perform relatively simple calculations. Because many of the problems included extraneous information,
Joseph had to decide not only the appropriate mathematical operations to use but also which numbers to
include in the calculation. She earned a standard score of 94 and a percentile rank of 34 indicating scores
in the average range.
Spelling measured Joseph s knowledge of prewriting skills and spelling words that were
presented orally. She received a standard score of 79 and a percentile rank of 8 indicating scores in the
limited range. Joseph spelled words in a non-automatic, laborious manner.

Passage Comprehension assessed reading comprehension and lexical knowledge. It required the
ability to use syntactic and semantic cues to identify a missing word in a text. Joseph earned a standard
score of 96 and a percentile rank of 40, indicating scores in the average range.
Calculation measured the ability to perform mathematical computations. The items required
Joseph to perform addition, subtraction, multiplication, division and combinations of these operations.
She received a standard score of 89 and a percentile rank of 23 indicating scores in the low average range.
Joseph solved initial problems quickly with no observed difficulties but demonstrated less automaticity
with the latter items.
Writing Samples measured Joseph s skill in writing responses to a variety of demands. The
sentences were evaluated for quality of expression. Errors in basic writing skills, such as spelling and
punctuation were not penalized. Joseph earned a standard score of 111 and a percentile rank of 77,
indicating scores in the high average range. She answered writing prompts with adequate, detailed
sentences.
Word Attack assessed the ability to apply phonic and structural analysis to pronounce nonsense or
low frequency words. Joseph was required to recall the phoneme (sound) associated with each grapheme
(letter) and then blend the phonemes into a word. She received a standard score of 65 and a percentile
rank of 1 indicating scores in the very low range. Low performance on this subtest may indicate that
Joseph may not have developed or mastered phonetic decoding skills.
Oral Reading measured story reading accuracy and prosody. Joseph earned a standard score of
84 and a percentile rank of 14, indicating scores in the low average range. Low results indicate that she
may have limited decoding skills, comprehension difficulties or both which result in a lack of reading
fluency. Joseph s errors consisted of substitutions (saying a different word than the one written),
mispronunciations and repetitions.
Sentence Reading Fluency assessed reading rate. It required the examinee to read and
comprehend simple sentences quickly. Joseph obtained a standard score of 79 and a percentile rank of 8,
indicating scores in the low range. She appeared to read sentences slowly, which may be due to limited
basic reading skills.
Math Facts Fluency measured math achievement and number facility. Joseph was required to
solve simple addition, subtraction, and multiplication problems rapidly. She received a standard score of
74 and a percentile rank of 4, indicating scores in the low range. Joseph solved problems at a slow rate
due to a lack of automaticity.
Sentence Writing Fluency assessed the ability to write rapidly with ease (automaticity).
Production of legible, simple sentences with acceptable syntax was required. Joseph earned a standard
score of 100 and a percentile rank of 50, indicating scores in the average range. Joseph worked slowly
but accurately, sustained concentration and wrote appropriate sentences.
Reading Recall measured reading comprehension and meaningful memory. Joseph earned a
standard score of 103 and a percentile rank of 55, indicating scores in the average range. She read at a
slow rate but recalled story details accurately.
Number Matrices assessed quantitative reasoning. Joseph was required to inductively and
deductively reason with numbers to determine a missing number in a matrix. She obtained a standard
score of 110 and a percentile rank of 73, indicating scores in the average range. Joseph worked slowly
but accurately.

Editing measured the ability to detect and correct errors in written work. Joseph was required to
identify and correct errors in punctuation, capitalization, spelling and usage in short written passages. She
received a standard score of 93 and a percentile rank of 31, indicating scores in the average range.
Word Reading Fluency assessed vocabulary knowledge and semantic fluency. Joseph obtained a
standard score of 88 and a percentile rank of 21, indicating scores within the low average range.
Joseph worked slowly and exhibited decoding difficulties.
Spelling of sounds assessed phonological and orthographic coding. Joseph was required to spell
nonsense words that conformed to conventional phonics and spelling rules. She earned a standard score of
83 and a percentile rank of 13, indicating scores in the low average range. This may indicate poor
phonological processing and lack of phoneme/grapheme knowledge.
Reading Vocabulary measured reading comprehension at the isolated word level. It required the
examinee to supply synonyms and antonyms. Joseph earned a standard score of 99 and a percentile rank
of 47, indicating scores in the average range.
Subtests were combined for cluster scores. A description and results follow.
The Broad Achievement cluster is a combination of the 9 subtests included in the Broad Reading,
Broad Mathematics, and Written Language clusters. This cluster represents Joseph s overall
performance across the various achievement domains. Joseph earned a standard score of 81, which
placed her in the 10th percentile indicating low average skills.
The Reading cluster, a combination of Letter-Word Identification and Passage Comprehension,
measures reading achievement or reading-writing ability including decoding and comprehension of
connected text. Joseph earned a standard score of 86, which placed her in the 18th percentile indicating
low average skills.
The Broad Reading cluster provides a comprehensive measure of reading achievement including
reading decoding, reading speed, and the ability to comprehend connected text while reading. LetterWord Identification, Passage Comprehension and Sentence Reading Fluency comprise this cluster. A
standard score of 81 placed Joseph in the 11th percentile indicating low average skills.
The Basic Reading Skills cluster measures sight vocabulary, phonics, and structural analysis that
provides a measure of basic reading skills and is a combination of the Letter-Word Identification and
Word Attack subtests. Joseph obtained a standard score of 69 and a percentile rank of 2, indicating very
low skills.
The Reading Comprehension cluster is a collective measure of comprehension and reasoning.
This cluster is a combination of Passage comprehension and Reading Recall. Joseph earned a standard
score of 98 and a percentile rank of 45, indicating average skills.
The Reading Fluency cluster provides a measure of several aspects of reading fluency including
prosody, automaticity and accuracy. Oral Reading and Sentence Reading Fluency subtests made up this
cluster. Joseph received a standard score of 79 and a percentile rank of 8, indicating low skills.
The Reading Rate cluster, a combination of Sentence Reading Fluency and Word Reading
Fluency, provides a measure of automaticity with reading at the single word and sentence levels. A

standard score of 82 was obtained which placed Joseph in the 12th percentile. These skills were in the
low average range.
The Mathematics cluster provides a measure of math achievement including problem solving and
computational skills. It is a combination of Applied Problems and Calculation. Joseph earned a standard
score of 90 and a percentile rank of 25, indicating average skills.
The Broad Mathematics cluster is a comprehensive measure of math achievement including
problem solving, number facility, automaticity, and reasoning. This cluster includes Applied Problems,
Calculation, and Math Facts Fluency. Joseph received a standard score of 83 and a percentile rank of 13,
indicating low average skills.
The Math Calculation Skills cluster assesses computational skills and automaticity with basic
math facts and it provides a measure of basic mathematical skills. Included in this cluster is Calculation
and Math Facts Fluency. A standard score of 79 was obtained placing Joseph in the 8th percentile,
indicating low skills.
The Math Problem Solving cluster, a combination of Applied Problems and Number Matrices,
provides a measure of mathematical knowledge and reasoning. Joseph earned a standard score of 102
and a percentile rank of 55, indicating average skills.
The Written Language assesses written language achievement including spelling of single word
responses and quality of expression. This cluster is comprised of Spelling and Writing Samples. Joseph
received a standard score of 92 and a percentile rank of 30, indicating skills in the average range.
The Broad Written Language cluster is a combination of 3 subtests: Spelling, Writing Samples
and Sentence Writing Fluency. This cluster provides a comprehensive measure of written language
achievement including spelling of single-word responses, fluency of production, and quality of
expression. Joseph obtained a standard score of 93 and a percentile rank of 33, indicating skills in the

The Basic Writing Skills cluster measures basic writing skills in both isolated and contextually

The Academic Applications assessed Joseph s ability to apply her knowledge and
comprehension skills in math, reading comprehension, and written expression measured by the Applied
Problems, Passage Comprehension and Writing Samples subtests. She received a standard score of 99
and a percentile rank of 48, indicating average skills.
The Phoneme-Grapheme Knowledge cluster is a combination of Word Attack and Spelling of
Sounds, requiring reading-writing and auditory processing abilities. It may be used to evaluate and
individuals proficiency with phonic generalization and her knowledge of common orthographic patterns
in both decoding and encoding. Joseph received a standard score of 71 and a percentile rank of 3,
indicating low skills.
Impressions
Based on personal interview, formal evaluation results, and informal observations, Joseph
presented with receptive and expressive language skills within normal limits. Her written language and
reading comprehension skills were also within normal limits. Joseph presented with academic skills in
the area of basic reading in the very low range. Joseph s academic fluency and academic skills were also
in the low range.
Recommendations
Speech-language therapy is recommended to address Joseph s reading and study skills.
Academic accommodations are also recommended and attached.

The following accommodations are recommended for Joseph in the academic environment.
1. Provide extended time of up to time and a half when taking test and exams.
2. Provide a quiet environment when taking tests and exams.
3. Allow computer-assisted technology when necessary.
4. Grade on content of answers. Do not penalize for spelling errors.
