help to take off the gown of drowsy patient with IV in situ because i didn't want to do any thing which may harm my client in any way. As
mentioned took help of family members to take care of patient at risk of fall, bed side mats were arranged for his safety and patient was
reinforced for calling before getting up from bed in week 12.

Navjyoti maintained attendance over the course of the 14 weeks of clinical and was always punctual. She arrived on time and was always
dressed in full uniform with her name badge visible to identify herself as a student. She demonstrated the ability to maintain competence
by utilizing her knowledge to provide safe competent and compassionate care. As well, she consistently demonstrated responsibility for
her actions by ensuring that she sought assistance when necessary. She maintained confidentiality by ensuring that her personal values
did not conflict with her practice. In addition, she demonstrated regard for commitments for self, peers and patients.

after 30 min again assessments i am able to enter response and evaluations as i did in my patient for week 9 after giving Hydromorphone
2mg PRN.

N.S. : yes, i do report to my instructor and nurse regularly. If their is a need of PRN medication i report to my instructor about the need,
also in week two my patient was not swallowing meds and i reported nurse and my instructor about it so that antibiotics can be given in
another form than orally by informing to physician. Physician was informed by the nurse and i documented it.If my patient refused to take
laxative medications , i informed my charge nurse and my instructor too in week 8.

first language punjabi to make thing more clear to clients and family members if their first language is punjabi too. I helped the Spiritual
helper in translating to punjabi patient on my floor, and also to a Social worker when a patient needed me to convey her full message to
social worker in English.Feel very good by helping anyone with language barriers.

Over the course of the semester, Navjyoti learnt to communicate effectively with her patients and with the inter-professional team. For
example, she ensured that she received a report of her patient's status every morning and learnt to explain to the patients the care she was
about to provide prior to initiating the actual care. Initially, she struggled with documentation but by week 8 she was documenting
accurately and concisely and in a timely manner. In addition, she developed the habit of maintaining eye contact with her patients during
conversations. Good job!

teach me. I need to improve by watching videos and doing more and more practice of IV medications.

N.S. : Week five my client was getting discharged and doctor's order said that same IV medication will continue at home and community
nurse will be calling them and doing their meds at home. Patient was unable to understand because of english not being their first
language and they misunderstood that have to go to some community nurse. I clarified them the whole situation and educated them about
the visit of community nurse.

Navjyoti demonstrated an appropriate relationship with her patients who welcomed the care she provided. She contracted with her teacher,
peers and inter-professional team when initiating patient care. Her behavior and attitude was appropriate in the clinical setting and in the
lab as well. She shared her knowledge with her peers, patients and back up nurse. For example, she was instrumental in assisting her
peers to program the IV pumps in the last few week of clinical. Good job!!

N.S. : I always take initiatives for learning new things, i went to nurse practioner and requested her to give me some information for the

helpful. I discussed the cardiac drugs in post conference and always with collaboration with my peers always ready for discussing new
topics for learning purposes . I always ask for new opportunities to learn IV medication and catheterization.

N.S. : I advocate for my clients. My patient was unable to swallow food and even was not accepting medications. family members were
forcefully feeding him and expecting a sooner discharge. I discussed the case with my primary nurse and my instructor so that patient can
be started with maintenance IV for medications and can be protected from getting malnourished because of inability to swallow. Primary
nurse called physician and discharge was postponed.

N.S. : Yes, i support clients rights for self-determination and choice by asking their needs and listening to them and understanding their
verbal and nonverbal signals and explaining whole process of my assessment to them.e.g if i take vital signs i always let them know their
findings to reduce the anxiety if patient is in condition to understand.While giving medication i tell them with colour difference the given
medication's purpose and how much it is given.

stroke. He was on chemical restraints and i lowered his bed to avoid any falls and removed any clutter around in the room to avoid any
injuries. My client in week 12 had a risk of fall and wanted to be in washroom alone, i involved family member for care and respected his
choice of being alone and in a professional way i reinforced my client for not getting up from the commode until i arrive after the bell.

function as a facilitator helping and guiding her peers to check and administer medication. She was instrumental in encouraging her peers
to share their knowledge and expertise with each other and by working together as a team. She demonstrated an increased knowledge and
understanding of her role as a student and functioned accordingly. She advocated for effective patient care as well, by ensuring that she
provides the best possible care and by assisting her peers as much as possible with safe medication administration and decision making.

and out of the rooms to search for equipment. She prepared two concept mapping satisfactorily to include interventions necessary to

N.S. : Yes I do my self reflection, for eg week two I was going to give insulin to my patient when my nurse told me his blood sugar level

improve my IV medication skills I am working to improving by seeing video and seeking for opportunity from my instructor.

N.S. : Didn't got chance to see many changes in health care system, we were shifted to oncology floor from palliative and here the pain
management was the priority and i focused always on doing pain assessment during my morning vital signs and also filled form for
assessing pain .Yes there are changes in health care system , which we can learn by quality assurance programs e.g recognising my
strengths and weakness and work to improve them.

To prioritize the needs of client i use my critical thinking and understand MAR instructions critical thinking is used. Blood sugar level of my
patient was 3.2 on day one and MAR sliding scale says no insulin if blood sugar is less than 4.0 so no insulin was given to patient.Also my
patients blood pressure was too low 76/54 mm of Hg and he was on amlodipine, which i had to hold ,on week 7 and charge nurse was
informed about that.

knowledge.

This semester,I am learning and getting experienced in giving oral and IV medication and learning new communication skills in hospital . Because of
the diverse population of patients in hospital I am getting much more exposure to patients of different cultures and hence learning to respect and
understand values and beliefs of different cultures..In the initial weeks i was not fully prepared to give medications , but with practice and with
guidance of our instructor, i could develop that confidence to give medications at right time and in organized manner.I learned to do IV medications
skill. I am getting more chances to apply my critical thinking. I am learning every week something new. Learned to make a care plan which is client
centered plan of care. It helps us to use our knowledge, skill and judgement skills . I get different experience with different patients every week.I am
getting more efficient in bed baths and client personal care. I want to practice more and more for giving IV medications, so that i can be perfect in
giving them by week 14. I worked on my time management and prioritizing the actions by making my to do's list for whole day in my clinical after
taking report which helped me a lot in organising my tasks and prioritising them as per the requirement. My another weakness where i want to
improve is documentation and i am working by charting the progress notes on rough sheet and showing it to my instructor. I will keep on watching
videos this semester to learn more and more and do my full readings.Need to learn skills for body mechanics and shifting patient comfortably without
hurting my back and without ant discomfort to patient,I will watch online videos for learning them.

Navjyoti is very knowledgeable and functions well in all aspects of patient care. She has successfully met most of the goals and objectives that she
planned to achieve in the beginning of the semester. For example, she was able to administer oral and subcutaneous medications independently by
week 14, and was able to administer IV medication with minimal assistance from her instructor. She sometimes struggled with the programming of the
IV pump when hanging a secondary line, but for the most part was able to prime the line and prepared and mixed the medication after having her
instructor double checking to ensure that she had the right medication for the right patient at the right time. As well, she keeps the primary nurse up to
date about the patients at all times, and she ensures that she gives a thorough report to her peers when going for break. She demonstrated the ability
to care for two patients and completed their care in a timely manner. Her documentation skills have improved as well, but there is still room for further
development. She was informed that her critical thinking, problem solving and decision making skills, physical assessment skills will develop overtime
as she continues to review and watch the nursing videos provided for her by the college. She was encouraged as well to review the document
published by the CNO on documentation standards.
