M.H. : I document clearly in the written form, as evident from my first documentation assignment. I have not gotten the chance to practice
documenting on the electronic method but look forward to doing so. I think I know how to document concisely and accurately as well according to the

M.H. : I have not gotten the opportunity to practice reporting to the appropriate personnel. However I do understand how to tell what information is

M.H. : I know how to communicate in an effective way towards my team members. I have only gotten the chance to work with classmates in a
teamwork setting, while in the stimulation labs. I look forward to working with different types of teams, and different team members as I continue my
education, and career.

M.H. : I haven't had the opportunity to really practice providing privacy and confidentiality. But I do understand that client nurse confidentiality is very
important to maintain throughout my practice. The patient is obligated to have their health kept private between patient and health care provider; no
one even a family member, unless otherwise told by the patient.

M.H. : In order to educate myself in the best way possible I must get and retrieve information from a variety of different resources. I know I have all
the online resources to help educate myself. I also have all my e textbooks which are very resourceful and rich of information. I also have recently
discovered the resoouces in the Humber library, and there are so many different resources available there.

Apply knowledge of the therapeutic nurse-client relationship and effective communication techniques furing the provision of nursing skills and
assessments.
M.H. : I know how to provide a therapeutic and professional nursing-client relationship while doing an assessment on a client. I have yet to practice
on a real client, but working with my classmates and practicing on friends and family I have developed these skills during certain nursing
assessments. I understand in order to provide these assessments I need provide honesty, equal respect and power, and an equal understanding
between myself, the nurse, and the client.

M.H. : I have establish a few meaningful and trusting relationships with those involved in my educational process. I have become close with some

M.H. : I have yet to use conflict resolution because luckily for me the people I have worked with are conflict free. But I do know how to implement
conflict resolution, as taught to me by my nursing theory class. I know I must try and gain an equal and collaborative understanding between the two
of us, that will work for the people involved in the conflict. And in order to gain a resolution we must try to avoid further conflict, if conflict is at risk of
getting worse we must step away from the issue and sort ourselves out.

M.H. : I know and understand that my behaviour can effect those around me. One small emotion, facial expression or mannerism can play a big role
in how others interpret my behaviors. And my behaviors can play a negative or positive role in other modes or emotions.

M.H. : I know everyone is different in their own way, whether they're different because of their culture, religion, or beliefs. I know I have to be aware of

M.H. : I know how to act in a way that will promote teamwork and positivist. I find if you try to stay positive those around you will as well, and

M.H. : While working with my peers we work collaboratively to try and better our skills with the help of one another. When something is done
incorrectly on my part my classmates will try to help me improve it and vice versa.

M.H. : I promote the idea of quality learning. Without a quality level of education I as a nursing student would not be as educated and informed as I

M.H. : I know every person is unique and has their individual needs and wants. I know when I begin my practice that I have to keep in mind each and
every person needs a different treatment and a different care. This is all based on the clients culture, religion, beliefs, and personal views.

M.H. : I know when looking at a client in a holistic view you must look at every aspect of them and their health, they are a whole person and you must
look at them in that way. When planning an intervention and assessment I know I must look at the persons, physical, metal and social well-being.

M.H. : I am still practicing my assessment and interviewing nursing skills, and I am gradually getting the hang of it. I know it will take time for me to
get a complete grasp of all these skills, but I have a firm understanding of how to implement all these skills.

M.H. : I have a clear understanding of how to preform a health assessment and nursing techniques in a safe and timely manner. I know that the
assessments should not risk the patient in any way. And it must not take up an excessive amount of time in order to not fatigue the patient.

M.H. : I have a good grasp and understanding of all these physical assessments. I know what procedures to do and what skills are needed for each
assessment. I still need more practice for each assessment, but my knowledge and understanding on each assessment are growing and developing
as I continue to practice.

M.H. : Assessment, Diagnosis, Planning, Implementation, Evaluation. Each part of the nursing process is very important in providing quality care.
Assessment is the overview of the entire patients physical and mental health. Diagnosis is the nursing diagnosis including the problem, etiology and
signs and symptoms. The planing is what the nurse will decide as an intervention process for the clients health issues. The implementation is actually
developing and acting out the plan. Lastly, the evaluation is the nurse evaluating if the nursing plan had an effect on the client's health issue.

Utilize the nursing process to meet clients healthcare needs related to hygiene, including integumentary care, nutrition, hydration, digestion,
elimination, mobility, and safety.
M.H. : I know when providing the client with the healthcare needs that I must take into consideration a variety of needs. I know there are specific
elements that need certain care for each individual client that I would need to keep track of to provide quality health care.

Discuss environmental strategies that promote health safety, and reduce risk including infection control, emergency codes, and ethically
appropriate and safe use of restraints.
M.H. : One of the most important parts in providing a safe environment is by making sure there is a good chain of infection control. The first step to
infection control would be good hand hygiene. I have been informed about the different types of security codes that may be used in a clinical setting
and I understand how to utilize these emergency codes. I also understand the appropriate times to use restraints on a patient.

Utilize infection prevention and control practice, including, but not limited to: hand washing and basic personal protective equipment (PPE).

M.H. : I understand the importance of infection control, and how the chain of infection control is very important to maintain, and at the same time very
easy to break. So at all times you must be aware of how to protect oneself from infection.

M.H. : Whenever I receive feedback from a classmate or teacher, I take it in and try to incorporate their feedback while I am practicing individually.

what I am capable of and where I can really improve.

M.H. : Before class I try to insure that I have completed everything that I must have before class time. If I dont I know I will be lost and out of focus. At
times I can slack a little on the preparation before class, but i try my best to be prepared to the best of my ability.

for certain clients, based on the needs they may have, as taught to us in theory class. I understand that different interventions will be needed, and the
choice of which interventions to use requires both decision making and problem-solving.

accordingly, based on their cultural beliefs, and what they are comfortable with.

For the past few months I have been enjoying every aspect of this program, and I have really enjoyed my learning experience. I have gained a lot of
knowledge in may areas, and have been gradually learning how to utilize all my recently gained knowledge. I know I still have a lot to improve on, and
I look forward to improving it all. I will continue to improve my skills by working hard, and practice in self teaching. I will do so by utilizing all my
resources weather it be the people around me or the data bases available. I am happy with everything so far. And I look forward to the rest of this
learning process.
