AFFIDAVIT
I, ERVIN V. TAGARI, of legal age, Filipino Citizen, Single and
with residence address at Barangay 2, Jones, Isabela, after
having been duly sworn to in accordance with law, do hereby
depose and state:
1.
That I am a Holder of a Philippine Passport No.
EB4947446 issued at the Department of Foreign Affairs (DFA),
Tuguegarao, Cagayan on March 14, 2012 and valid until March
13, 2017;
2.
That the validity of my said Passport is until March
13, 2017;
3.
That I have to renew my said Passport before the
end of its expiration or validity on March 13, 2017 since such
early renewal will cause a clean and new Passport;
4.
That I execute this Affidavit to attest to the truth of
the foregoing facts and for whatever legal intents and
purposes this may serve.
