FROM AWAY CONTEST OR EVENT

I, the parent or legal guardian of ______________________________________, will be transporting my

son/daughter home after todays event. I release the school and its coaches from all supervisory and
transportation responsibilities required for my child. I assume these responsibilities upon receiving my child
from the coach at the end of the event. I also understand that I need to have this form turned into the coach a
minimum of 24 hours prior to the start of the athletic event.
