MEMORANDUM OF UNDERSTANDING

KNOW ALL MEN BY THESE PRESENTS:

This memorandum of Understanding is made an entered into by and between:

The______________, represented in this Act by_____________), in his capacity as

the__________, of the __________ , hereinafter referred to as the FIRST PARTY; and
_________, Filipino, of legal age, Married, presently residing at ______________, Leyte,
and an employe of the _________________ , hereinafter referred to as the SECOND
PARTY.
WITNESSETH:
WHEREAS, on 14 April 2015, The President of the Republic of the Philippines
approved the grant of financial, who were affected by Typhoon Yolanda and the
7.2 Magnitude Earthquake;
WHEREAS, the SECOND PARTY was among the employees of the _______________
whose houses suffered total/heavy damages;
WHEREAS, the financial assistance in the amount of ____________ was approved for
release to the SECOND PARTY through the FIRST PARTY;
WHEREAS, the PSF assistance of Php _________ sahall be used by the SECOND
PARTY to reimburse/defray the acquisition of a new housing unit to replace the
washed-out housing unit or reconstruct/repait totally/heavily damaged housing unit.
NOW, THEREFORE, for and in consideration of the foregoing premises, the parties
hereto agree as they are hereby agreed to the following terms and conditions:
