SWORN DECLARATION/IDENTIFICATION OF RELATIVES BY CONSAGUINITY

I, AAA, Filipino citizen, of legal age, and presently residing at xxx, do hereby
declare/identify under oath that BAA, Filipino, of legal age, and residing at yyyn is
my SISTER or relative by consanguinity (individuals related by blood/second
degree), by virtue of our having the same mother, the late VAA, whose name
appears in the attached Certificate of Live Birth (Annex "A") of BAA and the also in
the attached MARRIAGE CONTRACT (Annex "B) of the affiant.
Also, I declare herein that I am the eldest daughter of VAA to her first
husband RBA, whose names appear in my attached Marriage Contract, a legal
document. And I also declare further in this document that I do not have a
Certificate of Live Birth currently on the records of the Government of the
Philippines (GOP) as the same was previously razed by fire in the Office of the Civil
Registrar.
Further, BAA is the daughter of my mother VAA with the latter's second
husband by the name of NAA as appearing in the attached Certificate of Live Birth
of the former. As such the affiant is her half-sister.
I declare under oath that this document has been accomplished by me, and is
a true, correct and complete statement pursuant to the provisions of pertinent laws
of the Philippines where this declaration has been sworn to. I also authorize the
herein attesting officer to verify/validate the contents stated herein, and to compare
the original of the documents attached here with their true copies. I trust that this
information shall remain confidential.
