JOINT AFFIDAVIT OF
TWO DISINTERESTED PERSONS
We, JISON B. NESNIA and RYAN D. VELANO, both Filipinos, of legal
age, single, and residents of Purok 3, Santa Cruz Viejo, Tanjay, and
Tubtubon, Sibulan, Negros Oriental, respectively, under oath, do hereby
depose and say that:
1. We personally know patient Rockie Yayong, Jr.;
2. When he requested for copy of his Certificate of Live Birth from the
National Statistics Office (NSO), we were surprised that his middle
name is DEGUIT and not ABUNIN;
3. We attest that his middle name is the one registered in the copy of his
certificate of live birth, with registry number 2008-244;
4. We are not related in any way, either by consanguinity or affinity, to
the said Rockie Yayong, Jr.; and
5. We are executing this Affidavit to attest to the truthfulness of the
foregoing and for whatever legal purposes this may serve.
IN WITNESS WHEREOF, we have hereunto set our hands this 10 th
