You, the Buyer, are contracting True Value Entertainment (company/DJ) for your musical
entertainment during your private event. On the day of TBD DJ will provide their Standard service at the
following location: TBD. During which the event will be held during the hours of: TBD to TBD . As for
legal consideration and for their service, the Buyer agrees to pay DJ TBD.

PERFORMANCE CONDITIONS
The Buyer will provide safe and appropriate working conditions. DJ requires a minimum of three 15-20amp circuit outlets from a reliable power source within 50 feet of the setup area. The Buyer lays
responsible for any delay in the performance or damage to DJ's equipment due to improper power. Where
available, please provide DJ and up to two personnel an editable meal.
This contract guarantees that DJ will be ready to perform at the start time of the event. No guarantee is
made as for the time of arrival; however, DJ requests that they be permitted 120 minutes before the event
and 60 minutes after the event for takedown.

MUSIC
DJ is not responsible if certain selections are unavailable. DJ will have special music requests if they are
received IN WRITING at least two weeks prior to the event. DJ will attempt to restrain explicit music;
however, DJ is not responsible for any complaints from such happenings.
DAMAGES
The performance of DJ is subject to proven detention by accidents, riots, strikes, epidemics, acts of God, or
any other legitimate conditions beyond their control. If such circumstances arise, all reasonable efforts will
be made to find a suitable replacement entertainment. Should DJ be unable to procure a replacement,
Buyer will receive a full refund within 10 business days. Buyer agrees that in all circumstances, DJ liability
will be exclusively limited to the equal amount of the performance fee, and DJ shall not be liable for
indirect or consequential damages arising from any breach of contract.
DJ is not responsible for any damages caused on the events premises/location or surrounding areas. As the
Buyer you are held liable for any injury or damages to DJ, or property of DJ, while on the premises -whether damage is caused by Buyer or guest, event invitees, event employees or any other party in
attendance, whether invited or not.
CANCELLATION
The Buyer and DJ agree that this contract is not subject to cancellation unless both parties have agreed to
such cancellation in writing 14 days before the event. Even in the event the Buyer breaches the contract, he
or she will continue to pay DJ the amount set forth above by/as "Performance Fee, -- as for the event date
was reserved in advance for your event.
DJ reserves the right, in good faith, to stop or cancel the performance should weather pose a potential
danger to him, the equipment or audience. All effort will be made to continue the event. However, safety
is predominant in all decisions. DJ's compensation will not be affected by such cancellation.
PAYMENT
DJ requires no deposit for events under $700. Payment is to be received the day of the event, or up to 5
weekdays after. If no payment is made DJ retains the right to collect though legal means. Buyer is
responsible for all court fees, legal fees and collection costs. Buyer shall be charged $30 for each bounced
check plus a $15.00 service charge for each collection notice. Credit/Debit cards are not accepted, and

checks must be made out to: Omar Blanco. If playing time exceeds the set time frame by ten minutes, a fee
of $85.00 per hour will be charged to the set performance fee commencing after the ten minute mark.
All sections contained on publication POETBG1403 Disclosure Agreement Policies govern this
document. All information within is confidential. No information will be disclosed without prior written
consent from each party.

PHOTOGRAPHY RIGHTS
Use of Likeness
I, TBD grant permission to utilize Events following, but not limited to: name, photograph,
likeness, voice, statements, personal experiences and recognizable imagery.

Photography Release Rights

I, Buyer, grant permission for True Value Entertainment to:
Photograph Event stated in the above Event Type and Location.
Publish Events photos in public print and digital form.
Release Events photos for processing or reproduction to third-parties such as: manufactures,
advertisers, distributors, vendors and/or agents.
Duplicate and distribute Events photos through any medium, including but not limited to:
merchandise, artwork, motion pictures, posters, social media and related mediums.
Have the unlimited permission to alter and modify Events photos.

Corse of Actions Rights

Buyer and Company as well as all event, related or non-related, employees and guests, invited or not:
Will refrain from any and all indirect claims, lawsuit or course of legal and non-legal actions against
True Value Entertainment, its employees, assigns, agents, licensees and affiliated third-parties.
Will dismiss any and all indirect charges for: negligence, defamation, invasion of privacy, right of
publicity, infringement of copyright or trademark, and violation of any other charge relating to
utilization of the rights granted under this contract.
Will make no attempt to restrain the use of any of the rights granted by this contract.
Agree to not go public in the event of a violation or breach of this contract.
All rights granted to Company are irrevocable and ongoing, and not subject to legal and non-legal rescission
or restraint under any circumstances. I understand granted rights include the ongoing worldwide use until
written termination. I agree to waive any right that I may have to inspect or approve any such use. Photos
taken during Event remain the copyrighted property of True Value Entertainment.
