AFFIDAVIT OF CONSENT
That I, HERMINIA CRUZ CATALAN, of legal age, Filipino,
married and a resident of Libueg, Camiling, Tarlac, after having duly
sworn in accordance with law, hereby depose and state:
1. That I am the paternal grandmother of the minor Ely Mae
Ibale, seven (7) years old, Filipino and living with me at
Libueg, Camiling, Tarlac, I being the mother of her father,
namely Ernesto Catalan also of the same address;
2. That the minor Ely Mae Ibales parents, namely Ernesto
Catalan and Marites Ibale, are not legally married;
3. That Marites Ibale is now living at Seattle, Washington, United
States of America;
4. That since the birth of the minor Ely Mae Ibale, she has been
in my care and custody to this day;
5. That as such I am the guardian of the said minor Ely Mae
Ibale;
6. That I hereby give my consent to the planned travel of the
said minor to Seattle, Washington, D.C., U.S.A. in order for her
to live with her mother Marites Ibale who is a permanent
resident thereat;
7. That I am executing this affidavit to attest to the truth of the
foregoing statements.
IN WITNESS WHEREOF, I have hereunto affixed my signature
