AFFIDAVIT OF CONSENT AND SUPPORT

I, ____________________, of legal age, Filipino, single and a resident
of ______________________ having been duly sworn in accordance with
law hereby depose and state that:
1. I am the mother of _____________________, born on

who is under my custody and currently residing with me at
the aforementioned address.
2. I hereby confirm my consent to my childs application for the
issuance of passport as well as to her travel to a foreign
destination.
3. As the parent of my minor child, I shall provide her with all the
financial support that she shall be needing while traveling
abroad, such as, but not limited to, payment of roundtrip
airfares, hotel or lodging accommodations, allowances for
food and other related expenses.
4. This Affidavit is being executed for purposes of obtaining a
valid passport from the Department of Foreign Affairs,
clearance from the Department of Social Welfare and
Development for her travel abroad, and for whatever legal
purpose this may serve.
