BIRTH OF CHANDRA SEKHAR KADIYALA

I, the undersigned, Nageswara Rao Kadiyala aged 61 years, Hindu by religion, presently
residing at 12-1-508/67 Lakshminagar, (via) N Lallaguda, Secunderabad, Telangana, 500017,
India, do hereby solemnly affirm and declare on oath that:
1. I was born in Irukupalem, Guntur district, Andhra Pradesh, India on Feb 1 st, 1954, and I
am the father of Chandra Sekhar Kadiyala.
2. I was married to Shantha Kumari Kadiyala on March 12th 1977.
3. Chandra Sekhar Kadiyala was born on 11-01-1981 to me and my late spouse, Shantha
Kumari Kadiyala in Hyderabad, Andhra Pradesh, India. I have the personal knowledge of
Chandra Sekhar Kadiyalas birth because I am his father, I took my wife (mother of
Chandra Sekhar Kadiyala) to the hospital and I was present at the time of Chandra Sekhar
Kadiyala's birth. I and my late spouse Shantha Kumari Kadiyala (mother of Chandra
Sekhar Kadiyala) are the natural birth parents of Chandra Sekhar Kadiyala, born from our
marriage.
4. Chandra Sekhar Kadiyala's birth was not registered because it was not customary at that
time to register the birth of children.
So I do hereby solemnly affirm that foregoing are true and correct statements.
