AFFIDAVIT OF BIRTH

I Ammaji Musunuru after being duly sworn do hereby depose and say that:
1. I am a citizen and national of India born on Aug 15th 1955 at Agiripally Krishna
district, Andhra Pradesh , India reside at Plot #673,Hmt Hills,
Kukatpally, Hyderabad, Telangana, 500085, India.
2. I have direct knowledge of the birth of Chandra Sekhar Kadiyala, as I am his Aunt
and was present at birth
3. Chandra Sekhar Kadiyala was born on Nov 1st 1981 at Hyderabad,
AndhraPradesh, India
4. Chandra Sekhar Kadiyalas natural parents are
deceased ShanthaKumari Kadiyala, the mother and Nageswara Rao Kadiyala, the
father.
5. This Affidavit is being submitted in support of the birth facts and parentage of
Chandra Sekhar Kadiyala
6. It is my prayer that this Affidavit be accepted for this purpose.
