The!undersigned!met!with!an!accident!in!due!performance!of!his!official!duties!
on!17th!March!2015.!
The!details!of!the!accident!and!disability!are!given!below:!
The! undersigned! was! doing! internal! audit! of! Impetus! Section! at! Bengal!
Chemical! Bhavan,! Prabhadevi! in! terms! of! IOM! No.! WO/Audit!
Comm/IMPETUS/2/1118/2014X15/53! dated! 27th! Feb.! 2015! (copy! enclosed).!
Due! to! audit! timelines! and! exigencies! of! service! of! the! company! the!
undersigned!was!working!till!late!at!the!office!premises!of!the!organization!and!
left!office!at!8:30!pm.!However,!the!undersigned!fell!from!the!stairs!on!Ground!
Floor! of! Bengal! Chemical! Bhavan.! It! is! also! brought! to! notice! that! there! is!
inadequate!lighting!at!Ground!Floor!stairs!of!the!building!besides!the!fact!that!
the!stairs!are!of!black!colour.!
The! accident! was! immediately! reported! by! undersigned! to! the! controlling!
officer! (Regional! Audit! Head)! and! on! next! day! morning! to! the! audit! team! inX
charge! (DGM! Drilling,! Internal! Audit! Mumbai)! through! phone! calls.! The!
undersigned!was!helped!by!the!security!personnel!at!the!building!in!arranging!
taxi! for! residence.! The! accident! resulted! in! serious! bodily! injuries.! On! 18th!
March! 2015,! the! undersigned! went! to! Hinduja! Hospital! and! consulted! the!
orthopedic!surgeon,!Dr.!Vivek!Shetty.!The!doctor!diagnosed!multiple!fractures!
of! left! hand! wrist! joint! and! injuries! to! rib! tendons.! The! left! hand! of! the!
undersigned! was! plastered! with! one! week! of! soft! plaster! and! thereafter! five!
weeks! of! hard! plaster.! Due! to! his! permanent! physical! disabilities,! the!
undersigned! walks! using! artificial! leg! at! office! and! crutches! at! home! (copy!
enclosed).! However! due! to! the! multiple! injuries! suffered! in! hand! and! ribs! he!
was!not!in!position!to!walk!using!artificial!leg!or!crutches.!The!doctor!advised!
bed!rest!from!18th!March!2015!to!29th!April!2015!in!view!of!the!multiple!
