2 That being employee of the different hospital/company I was issued
and holder possessor of the following certificate, to wit:
a. CERTIFICATE OF EMPLOYMENT issued by Dr. Almozher
Polyclinic on December 9, 2013;
b. CERTIFICATION issued by the Benguet General Hospital, La
Trinidad, Benguet issued on May 12, 2010; and
c. CERTIFICATION issued by the Philippine National Red Cross,
Benguet Chapter;
3 That I hereby attest to the authenticity and genuineness of the
document/s attached made an integral;
4 That I hereby agree to be liable and answerable to perjury and/or
falsification of public documents in the event that the document/s
is/are found not in order;
5 That I am executing this affidavit to attest to the veracity and
truthfulness of the foregoing and for all legal intents and purposes
it may serve.
