You are hereby commanded to produce and permit inspection and copy at 717 S.
Houston, Ste. 500, Tulsa, OK 74127, in the County of Tulsa, State of Oklahoma on the
28th day of April, 200 of the following designated documents, records or tangible things
in your possession, custody, or control.

All records, documents, information, investigative reports, notes,

supplemental reports, statements, summaries, affidavits, photographs, diagrams,
audio video and any other electronic or mechanical recordings ( regardless of
format be it CD, DVD, Computer disc. This includes still captures, stop action, and
full action), 911 tapes, medical records, and any and all documents regardless of
type or form relating to:
