Greetings!
Please be informed that the undersigned tried to comply the requirements
pertaining to BIR requirements specially the transferring of BIR Form 1905 from RDO#
49 with registered address at North Makati BIR Regional Office 313 sen gil 5 Puyat
Ave., Makati City to RDO# 050 with registered address at South Makati 5 th/f Atrium bldg.
Makati Ave., Makati City. However, upon inquiry on the BIR office which is situated at
Barangay Casisang, Malaybalay, City, Bukidnon it advised the undersigned that it
cannot process such transfer since the undersigned has been registered a taxpayer at
Makati City, where the previous employer (LBC INC. Express) had remitted its tax
payments withhold from the undersigned. Be it noted that the undersigned is previously
employed at LBC Inc. Express assigned at Malaybalay City, Bukidnon.
Thus, the undersigned respectfully request/allow any of its RCBC Human
Resource Personnel or any of its authorized employee to transfer/process such form
from BIR RDO# 49 to BIR RDO# 50 with full power and authority whatsoever requisite
or necessary or proper to be done in and about the premises as fully to all intents and
purposes as the undersigned might and could lawfully do if personally present, with
power of substitution and revocation, and hereby, ratifying and confirming all that the
RCBC personnel or his substitute shall lawfully do or cause to be done under and by
virtue of these presents.
