WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.

What are the schools injury policies? (e.g. What should you do if a child gets
injured?)
o Send them to their cooperating afterschool teacher who will help them
or send them to the nurse
What documents must you complete?
o None
Where is the first aid kit?
o In the gym with the afterschool program or with the nurse
Who should you contact in the event of an injury?
o I should contact their afterschool teacher

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures?


o In case of a fire, the procedure is to bring all the students to the grass,
if it is safe, and have them line up
Where are the fire exits?
o Go to the open grass
Where is the meeting point?
o Grass field, or black top if necessary

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children?
o Their individual should not have access to children ulness they are
apart of the afterschool program.

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted?

ABOUT THE ACTIVITY SPACE:


A. Risk assessment of activity space: Identify all potential hazards in the
OUTDOOR activity space (draw a map of the area and indicated all potential
hazards).
On the blacktop, the random poles could be hazardous in activites that
involve chasing or fleeing, the benches near the side could also be
hazardous. The grass area has some holes, or uneven areas that could be
dangerous
B. Risk assessment of activity space: Identify all potential hazards in the
INDOOR activity space (draw a map of the area and indicated all potential
hazards).
There are a lot of tables within the gymnasium that will need to be pushed
to the side to avoid injury
C. Where are the closest restrooms and water fountains?
Both the bathroom and water fountain are right outside the gymnasium
close to the basketball courts. There is also a bathroom in the teachers
lounge if needed.

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs.
- Be aware of student profile/medication and identify school medical
personnel responsible for these students

PRE-PROGRAM PREPARATION:
A. Where is the equipment located and how will I access it?
Some equipment will be brought from Pacifics campus, and there is extra
equipment available onsite located in the shed on the blacktop. The key is
in the gym with the sign in binders.
B. Where will I go if it rains?
If it rains, the lesson will be moved inside to the gymnasium and the tables
will be moved to the side

How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)?
o I will make sure on hot days to have the activities in shaded areas
as best as possible, also allowing for more water breaks on days
where it is hot.

What will I do if a student is not appropriately dressed for Physical


Education?
o The student will either be asked to sit out due to safety
considerations or the activity will have to be modified to meet the
students needs

Where can I leave my personal belongings?


o On the benches near the basketball courts where the camera and
equipment will be.

What access, if any, do I have to the teachers lounge/facilities?


o If needed we can use their bathroom facilities in the teachers
lounge
