The Dane County Human Services Department has many different agencies that provide
services to children and families of Dane County. There are also small suburban communities
that offer services. The following is a list of providers:

Scoring Guide Criteria

Self-Score

Resource file includes a minimum of 25 different agencies or


organizations that may provide services or information and support
for children and families.
Each agency listed includes name, address, phone number, email,
and a contact name (when possible).
Listing for each agency is accompanied with a brief description of
services provided and eligibility requirements.
Resource file includes a minimum of 10 brochures from the
agencies listed in the file.
Resource file identifies a minimum of five local community
agencies or organizations (others may be state or national
organizations).
At least 20 listings include websites with the name of the website
and the link address
Resource file covers a variety of services for children and families,
and adheres to principles of anti-bias and respect for diversity
Resource file is neat and well organized, with agencies grouped
under headings by type of services provided.
Resource file applies standard rules of language structure
including grammar, spelling and punctuation.
Resource file is submitted electronically

Scoring guide and self-assessment is included with submission of


assessment. (Copy and paste this scoring guide or save it and
type in this document. Highlight, bold, or type in the number you
