As I completed my first year, I notice that students with disabilities werent receiving enough physical activity throughout the
week. Students were getting less than 60 minutes of appropriate physical activity throughout the week. The students only have
Adaptive Physical Education twice a week and usually get an average of 20 minutes of moderate physical activity.

I am currently in my second year of teaching in the Shorewood Area School district. I am teaching at Shorewood Intermediate
Middle School. The school is located in an urban location and has approximately 300 students. In the school 25.2 % of the
students are at an economic disadvantage and 74.8 % of students are not. The schools Race and Ethnicity rates are 65.4% White,
14.2% Black, 6.8% Asian, 5.2 % Hispanic, and 8.1% mixed. We have a low percentage of students with disabilities at 13.6%, but
the rates has been increasing lately.

Based on your reflection, describe the reason for your goal. Describe how your goal is linked to your educational situation, and
write out the Wisconsin Educator Standards (two or more) that you will focus on to meet your goal.
The Shorewood district places strong emphases on the utilization and production during class time. Due to the increase of obesity
and lack of activity time in the special education area, new and innovative programs are being developed on a continuing basis. I
plan to find innovative methods to maximize physical activity during the school weeks. I will use pedometers, heart rate
monitors, and other innovative ways to measure my students activity levels. Using technology allows me to measure student
performance and gives the students a concrete visual realization of their performance. I will focus on Wisconsin standard 5
teachers know how to manage a classroom. By focusing on individual and group motivation it, will create a beneficial learning
environment which will allow my students to actively engage in learning aiding in their self- motivation. I will also focus on
Wisconsin Standard 6 Teachers communicate well. By using multiple types of technology to teach it will foster active inquiry,
collaboration, and will support positive interactions in my classroom. It will keep my students interest by varying my instruction
and will produce effective verbal and nonverbal communication.

I will assess my professional growth by creating a student satisfaction survey that parents will complete and return. By
learning how my students parents feel about their students activity level it will help me gauge my productivity throughout the
course of the year. To assess my students learning I will use pedometers and heart rate monitors to measure their performance
levels and overall time in moderate physical activity. This will assess my students understanding of the importance of being
physically active.

Step III: Annual Review of the PDP
Annual reviews are done in years two, three, and four and will be submitted as part of your completed plan in the last year of your
licensure cycle.
Return to Step II E and enter completion dates for each activity completed.
Professional Educators must complete a five-year plan with three annual reviews.
Write a reflection in year two summarizing the progress made towards completing your goal.
Indicate, in your reflection, how you grew professionally and/or how your professional growth had an effect on student learning.
Write any revisions to your objectives or activities in the revision area.
Return to this step annually until your PDP is complete and ready to be submitted for verification.
Initial Educators may complete a 3, 4, or 5 year plan. One annual review is required for a three-year plan, two annual reviews are
required for a four-year plan, and three annual reviews are required for a five-year plan. If there is a substantial change to your GOAL,
you must complete and resubmit section II A-II E to a team for review and approval of the new goal.
