Identify a student whose learning would be enhanced by collaborative efforts and provide a rationale for why this student was selected. (Use only
the students first name.)
I have chosen a student named Adam. Adam is a kind student and is very cooperative. I have him all year for ENG III A & B. He is a junior and his
mother has expressed concerns for his life after high school due to his struggles with organization. I want to collaborate with Adam and help him
develop organizational skills that he can use now, in college, and in the future workforce. He also has a year to complete his requirements for his
Eagle Scouts, so I think this could be a real-world applicable opportunity for him to implement his organization skills. Since I have Adam all year,
this will allow me to collaborate with him consistently and track his progress.
Describe your plan for contacting and developing the collaboration plan with the parents/caregivers.
I have been in communication with the student's mother throughout the school year to develop a plan. I have also talked to his case load manager,
Paula Thomason. Additionally, I have communicated with his other teachers as well.

1) Adam will have an organized agenda book with daily assignments.
2) Adam will create a calendar with all major assignments/tests.
3) Adam will organize his non-traditional work in a separate document.
4) Adam will create a plan for how he will finish his Eagle Scout requirements within the year.

Assessment Plan How will you measure the impact of the collaborative effort on student learning?
I will keep in contact with all of Adam's teachers to see if his scores improve once he is well-organized.

Describe the progress made in your collaborative efforts providing appropriate documentation.
This student has made great strives in his endeavours to turn in assignments on time. They had a high success rate when turning in
assignments for Non-Traditional Snow Days. He needs to do better with the agenda book. He's okay with writing down the big
assignments, but he's still having trouble with the daily work. The same goes for the Eagle Scouts--he knows what needs to be done
within a year, but he has a hard time pacing himself with the daily work to get there. Mrs. Thomasson is helping him more with his
agenda book.
His mother reponsded to a quiestoinnaire that was sent hoe and it provided good direction as to what Adam would like to accomplish. I
also received responses froom some of Adam's teachers/caseworkers. I have worked with Adam on using his agenda book more
regularly. He is good at writing down the dates of major assignments and tests, but he could be better at writing down his assignments

daily.
I am currently working with him on his plan for fnishing his Eagle Scout requirements.
Adam and I created his NT day plan and had it signed by all his teachers. He successfully turned in his NT Day 5 assignments.

Cycle 3 Committee Meeting


Using the assessment data collected, describe the impact of the collaboration plan on this students learning and describe possible next steps.
Adam continues to succesfully use his agenda book to document what homework he needs to complete. I have seen a large decrease in the
amount of missing assignments.
He also uses his calendar with large assignments to remember when his tests are coming up.
He succesfully submitted all nontraditional assignments for NT day 5 in to his teachers, using the plan that was signed by all his teacehrs.
His mother said that this plan worked succcessfully for him.
I have been in regular contact with his parents and with Mrs. Thomason to update them of his progress.
Unfortunately, he did not comlete his boy scouts plans. While he has gotten better at turning in assignments, he has not gotten better at
taking notes during class and completing assignments while in class. Collaboration efforts were unsuccessful in this area. His
organizational skills have improved, but his ability to stay on focus has not. I hope that next year I can develop a plan to help him work
on this.
