REASON FOR REFERRAL
Georgina Waldorf was referred for a Rorschach Technique Evaluation by her
husband and physician, John Waldorf. The test was administered to
determine Georginas mental stability and interpretations of physical objects.
The Minnesota Multiphase Personality Inventory was also recommended. The
MMPI is used to determine and track the effectiveness of current treatments
being prescribed.
GENERAL OBSERVATION
At first, Georgina looked a bit confused. As she started speaking to me (or
herself), she looked tired. The more she went on speaking, the more
exhausted she started to look. Georgina sighs very occasionally. She dragged
on her words and she rambled quite a lot.
As we conversed, Georgina took a while to think of an answer before she
retorted. She looked like she was protecting a secret no one knows. She
slowly gazed around the room as if she was looking for something in
particular. The sound of her husbands name turns her facial expression into
a slight scowl.

FAMILY HISTORY
Georgina Waldorf has a husband named John, who is also a physician.
Georgina and her husband own a home, but they are temporarily staying at
another house due to the current remodeling and reconstruction. Their
marriage is currently stable. Georgina and her husband have a newborn baby
boy, who is being cared for by a nanny named Mary. Only John, Georgina,
and Johns sister reside in the temporary home. Johns sister is also the
housekeeper. She has a few close relatives: Henry, Julia, Nellie, and Jennie.
EMOTIONAL DISORDER HISTORY
Georgina Waldorf said that her husband had diagnosed her with temporary
nervous depressiona slight hysterical tendency. Georgina is currently
taking both phosphates and phosphites. Georgina has been prescribed tonics
and journeys. She was encouraged to go out for journeys to catch some air.
She was also encouraged to daily exercise, consumption of cod liver oil, ale,
wine, and rare meats.
FUNCTIONAL LIMITATIONS
Georgina Waldorf cannot participate in any physical or mental activities. She
cannot strain herself no matter how small of a degree.
She is allowed to dress herself as well as entertain herself and others. She
can also order things.
DIAGNOSIS
A diagnosis has been found after consecutive rorschach tests and the
Minnesota Multiphase Personality Inventory. Georgina Waldorf has been
diagnosed with the case of hallucinations due to her current situation.
Hallucinations involve sensing things while awake that appear to be real, but
instead have been created by the mind.
